- Solicitation Number: VA259-14-I-0473 Notice Type: Sources Sought Synopsis: SOURCES SOUGHT * This is an amendment from the previous RFI - Sources Sought posted on May 13, 2014. This amendment is based on a NAICS change from 621610 to 532291. The response time has been extended to Jun 6, 2014 at 12:00 p.m. The purpose of this announcement is to gain knowledge of potential qualified sources and their size classification (hub-zone, 8(a), small, small disadvantaged, service disabled veteran owned small business, veteran owned small business or large business) relative to NAICS Code 532291 (size standard of $7.0 million). Responses to this notice will be used by the Government to make appropriate acquisition decisions. The Department of Veteran Affairs, Network Contracting Office (NCO) 19, anticipates awarding multiple contracts to provide Durable Medical Equipment (DME) Services to the beneficiaries of the following six (6) different VA Medical Centers and Health Care Systems within the Veterans Integrated Service Network (VISN) 19 geographic area. VISN - 19 Medical Centers and Health Care Systems are listed below: Cheyenne VA Medical Center (Cheyenne, WY) 2360 East Pershing Blvd Cheyenne, WY 82001 Denver VA Medical Center and the Eastern Colorado Hospital Care System (VAECHCS) 1055 Clermont Street Denver, Colorado 80220 Department of Veterans Affairs Salt Lake City Health Care System (VASLCHCS) 500 Foothill Drive Salt Lake City, UT 84148 Grand Junction VA Medical Center (Grand Junction, CO) 2121 North Avenue Grand Junction, CO 81501 Sheridan VA Medical Center (Sheridan, WY) 1898 Fort Road Sheridan, WY 82801 VA Medical Center and Ambulatory Care Clinic (Ft. Harrison, MT) 3687 Veterans Drive, P.O.BOX 1500 Fort Harrison, MT 59636 The intended contract period will be for a Base and four (1) year options. Summary of Products and Services to be solicited: Contractor/s shall provide all supplies, materials, equipment, labor, supervision, transportation, infection control, storage, cleaning, delivery, sanitization, assembly/set-up, repair, pick-up and patient training/instruction-in-the-use of Government-owned DME. Contractor/s must have the capabilities of providing 24/7/365 DME Services to include the delivery, set-up and maintenance of equipment to meet beneficiary's needs at his/her place of residence, and the education of the beneficiary in the use of the DME. Cleaning, sanitizing, and disinfecting DME equipment by approved methods must be provided by the contractor/s and equipment shall be kept in first-class operating condition in accordance with the manufacturer's recommendations. The government furnished DME may include, but is not limited to: hospital beds, mattresses, side rails, patient lifts, trapeze bars, wheelchairs and scooters. All equipment shall be furnished by and will remain the property of the VA Medical Centers. The area of service shall be all beneficiaries serviced by each of the VISN 19 VA Medical Centers, including surrounding areas, regardless of where the beneficiary resides. Should a veteran(s) be located within a zone that is not listed for that particular VA Medical Center, but is within a zone listed through a separate VA Medical Center, the Government reserves the right to purchase the services from the contractor awarded the contract for the zone that the beneficiary resides in. Specific geographical boundaries and areas of responsibility/jurisdiction for the VA Medical Centers are shown in the attached map showing the states and counties covered by VISN 19.
