- The VA White River Junction Healthcare System (VAWRJ) located at 215 North Main Street, White River Junction, VT, intends to negotiate for Deicing Road Salt Delivered to the VA as stated in the following Statement of Work as a sole source contract to Barrett Trucking Co., of Burlington, VT 05401. Statement of Work A. GENERAL: 1.The contractor shall provide 400 Tons of Deicing Salt With a moisture content of no more then 1% delivered to the building 43 as requested over the period of the Contract from 10/01/2014 through 9/30/2015. 2.LOCATIONS OF SERVICES PROVIDED: VA Medical Center, 215 North Main Street, White River Junction, VT 05009. B.WORK HOURS: 1. Normal Work Hours: The service schedule will be developed between the contractor and Contractor's Technical Representative (COTR) prior to any service being performed. 2. National Holidays: The ten holidays observed by the Federal Government are: New Years Day, Martin Luther King's Birthday, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran's Day, Thanksgiving Day and Christmas Day and any other day specifically declared by the President of the United States to be a national holiday. C.SPECIFICATIONS: 1. The contractor shall conform to all federal, state, and local regulations governing examining and licensing of pest control operators, performance of pest control, use of approved pest control chemicals and equipment, and proper disposal methods for all products/containers, which may be in effect for the area where the work under the contract will be performed. D. CALL BACKS: 1.EMERGENCY CALL BACK: NA 2.NON-EMERGENT CALLS: NA 3.E. REPORTING FOR SCHEDULED SERVICES: When Salt is delivered the driver shall go to building 43 and dump the product into the second bay from the right. The bay size is 13 feet 6 inches wide, and 12 feet 2 inches high. Vehicle shall be able to dump a 22 ton load at the door. After dumping the load of salt Contractor shall stop at building 10 and leave the invoice. F. SAFETY FACTORS: Salt shall be dumped into 2nd bay from the left side of the building. Grounds crew will safely push salt in with their equipment. G.STORAGE OF Deicing Road Salt: Salt will be stored in building 43 the 2nd bay from the left looking at the building. H.QUALITY ASSURANCE: Performance ObjectivePerformance Threshold All emergency Deliveries will be addressed and corrected within 2 days. Non-emergent complaints will be addressed and corrected within 1 week.Failure to meet prescribed timeframes shall not exceed 3 per Season. I. Required Contractor Reporting 1.Contractor will let Grounds know when Salt will be delivered by calling 1-(802)-295-9363 ext 5908 if no answer they can leave a message. J. CONTRACTOR PERSONNEL BADGES AND PARKING: 1. The contractor shall be required to comply with all security policies/requirements of The VA at White River Junction. All security policies/requirements must be met and employees cleared prior to the contractor performing work under this contract. Employees that cannot meet the security and clearance requirements shall not be allowed to perform work under this contract. K. INTERFERENCE TO NORMAL FUNCTION: Contractor may be required to interrupt their work at anytime so as not to interfere with the normal functioning of the facility, including utility services, fire protection systems, and passage of facility patients, personnel, equipment and carts. 1.In the event of an emergency, contractor services may be stopped and rescheduled at no additional cost to the government. 2. Contractor personnel shall inform the COTR or the designee of the need to gain access to secured areas. If access is required to secure areas, prearranged scheduling will be made with COTR or designee. This Notice of Intent is NOT a request for competitive proposals. Parties may identify their interest, accompanied by supporting literature, to the Contract Specialist no later than 05/08/2014 at 2:00 p.m. Responses should be sent to Mary.Bosch@va.gov. Information received will be for the purpose of determining whether or not to conduct a competitive procurement. The determination to conduct a competitive procurement based on responses to this notice is solely within the discretion of the Contracting Officer.
