- Solicitation Number: W912TF-14-R-0004 Notice Type: Pre-solicitation Synopsis: This is a pre-solicitation synopsis for a commercial services prepared in accordance with the format in FAR Subpart 5.201, for the purposes of publicizing this proposed action and is not to be considered a request for proposal. The New Hampshire National Guard (NHNG) is seeking to contract for care coordination and referral services to members of the US Armed Forces as defined in 10 USC 10(a) (4.) and (c) and their families within the state of New Hampshire. The goal of the program is to enhance the retention, readiness and reintegration of service members and their families. The beneficiaries of the Care Coordination Program are all National Guardsmen, Reservists, and Active Duty Service Members and their families who are geographically separated from military installations and live or work in the State of New Hampshire. For data collection and historical reporting, this population is divided into service members and families who are within the timeframe of the 4-stage deployment cycle as defined by Public Law 110-181 and those who are outside this period. The NACIS Code for this requirement has been determined to be 624190, Other Individual and Family Services with a small business size standard of $10.0 million. THIS REQUIREMENT IS BEING SET-ASIDE FOR SERVICE DISABLED VETERANS OWNED SMALL BUSINESS (SDVOSB) FIRMS ONLY. A single award, Firm Fixed Price contract is anticipated from a competitive Request for Proposal (RFP). The period of performance is anticipated to be a base year and four (4) one (1) year option periods for a total of 60 months, if the options are exercised. Place of Performance: The location of the work is throughout the entire state of New Hampshire. It is anticipated that the final RFP will be posted under solicitation number W912TF-14-R-0004, which will be made available electronically on FedBizOpps and the Army Single Face to Industry (AFSI) website https://acquistion.army.mil/asfi/default.cfm upon it release. At this time the solicitation is expected to be released on or about 13 June 2014. Interested parties are solely responsible for monitoring the above mentioned website for posting of the solicitation and any amendments thereto. The closing date of the RFP will be established in the RFP. Offerors must be registered in the System for Award Management (SAM) database to be considered for award.
