- This is a combined synopsis/solicitation for commercial services identified below, prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested. Solicitation VA-263-14-Q-3657 is being issued as a Small-Business set-aside Request for Quotation (RFQ) and incorporates provisions and clauses that are in effect through Federal Acquisition Circular 2005-74. The North American Industry Classification System Code (NAICS) is 333921 with the small business size standard of 500 Employees. The Iowa City VA Health Care System located in Iowa City, Iowa, has a requirement to procure commercial elevator maintenance services for a Base Year period 07/01/2014 - 6/30/2015 with four (4) Option Years. Contractor shall provide all necessary supervision, labor, equipment, materials and parts for inspection, preventative maintenance and emergency repairs and safety tests of the designated equipment. All work performed under this contract shall be in accordance with accredited standards and codes applicable to the equipment covered under this contract as required and outlined by VHS&RA Supplement, MP-3, G-series Guidelines and accordance with attached special conditions. See attached Scope of Work and equipment lists. This solicitation incorporates the following FAR clauses, provisions and addendums: 52.212-1, Instructions to Offerors- Commercial Items; 52.212-3, Offeror Representations and Certifications-Commercial Items 52.212-4, Contract Terms and Conditions-Commercial Items 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items; and 52.203-17, 52.204-9, 52.217-8, 52.217-9, 52.228-5, 52.232-39, 52.232-40; and the following VAAR Clauses and Provisions; 852.203-70, 852.215-71, 852.232-72, and 852.237-70. All hours worked on this contract will be under Service Contract WG 06-0035 (Rev.-9) 1/28/2014 for Elevator Services. The Government shall award a contract resulting from this solicitation to the responsive/responsible offeror/s whose offer conforming to the solicitation will be most advantageous to the Government per FAR 13.106-2. Responses shall include descriptive material and other such information corresponding to each minimum required item, which demonstrates the Offeror's capabilities, and shall identify where the offered service meets or does not meet each of the Governments' functional and performance minimum specifications. Information concerning clauses and provision incorporated by reference may be obtained at http://www.acquisition.gov. Submit three references along with pricing proposal for Base Year and 4 Option Years. Quotation is due by 4:00 pm CT on Monday, June 23, 2014, with anticipated award date of 6/27/2014.
