- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (1) The solicitation number VA259-14-Q-0541 is issued as a request for quotation (RFQ). (2) This solicitation is set-aside for 100% Small Business under NAICS code 335410. (3) Contract Line Item number:1 (4) Description of requirements for the items to be acquired: Line Item Product#DescriptionQty 1VH-2400119706 Vasoview 6 ProPkg60 includes trocar; blunt tip; cannula; 5cc syringe; 30cc syringe; and conical Dissection tip Item Specifications and Terms of Delivery Endoscopic Vessel Harvesting (EVH) System 1. Background EVH Procedure EVH stands for Endoscopic Vessel Harvesting. During a Coronary Bypass procedure, one or more healthy blood vessels will be taken (or "harvested") from the patient's leg, arm or chest to use as "new" vessels for bypass grafts. There are three techniques for harvesting a vessel for a CABG procedure: open, bridging and EVH. EVH uses special instruments to view and remove a blood vessel with much less trauma to the vessel itself or to surrounding tissues than bridging or an "open" procedure. In the past, one long incision was made from the ankle to the groin; this procedure is called an open procedure. It is highly invasive, often caused patients more pain than their chest incision, and resulted in a long scar. An alternative, less invasive technique to the open procedure is called "bridging". Bridging enables harvesters to gain access to the saphenous vein through three or four smaller incisions of about three inches. EVH is minimally invasive and uses special instruments to view and remove the blood vessel. EVH usually only requires one small incision about one inch long. The EVH procedure is performed immediately before heart surgery and can be used for the following vessels: The greater saphenous vein, which runs the length of the leg; the radial artery, which runs from the wrist to the elbow in the arm. There is generally little patient discomfort from endoscopic vessel harvesting, especially when compared to open or bridging procedures. In clinical studies, some important benefits have been shown in EVH including: oSignificantly less risk of infection and wound complications oLess postoperative pain and swelling oFaster recovery with minimal scarring oGreater patient satisfaction The VASOVIEW 6 PRO System is indicated for use in minimally invasive surgery allowing access for vessel harvesting, and is primarily indicated for patients undergoing endoscopic surgery for arterial bypass. It is indicated for cutting tissue and controlling bleeding through coagulation, and for patients requiring blunt dissection of tissue including dissection of blood vessels, dissection of blood vessels of the extremities, dissection of ducts and other structures in the extraperitoneal or subcutaneous extremity and thoracic space. Extremity procedures include tissue dissection/vessel harvesting along the saphenous vein for use in coronary artery bypass grafting and peripheral artery bypass or the radial artery for use in coronary artery bypass grafting. Thorascopic procedures include exposure and dissection of structures external to the parietal pleura, including nerves, blood vessels and other tissues of the chest wall. Device Description: The VASOVIEW 6 PRO Cannula The VASOVIEW 6 PRO Harvesting Cannula (Figure 1) is designed for use in conjunction with the 7 mm Endoscope. The Harvesting Cannula has four lumens which house the Endoscope, C-Ring, distal lens washer tube and Bipolar Bisector for ligation and division of vessel branches. The C-Ring/distal lens washer is independently controlled by a C-Ring Slider on the handle of the device for retraction of the vessel as well as washing of the distal tip of the endoscope. The Bipolar Bisector can be extended/retracted/rotated with the Bisector Carriage. Mechanical cutting is achieved with the Bisector Toggle located on the Bisector Carriage. Bipolar coagulation is achieved using electrosurgical energy. Positioning of the device, coagulation, and mechanical cutting are performed under endoscopic visualization. This device is intended for use with the bipolar outputs of compatible generators. Item Specifications: Stock No. VH-2400, Item No. 119706 - VASOVIEW 6 PRO - This product is sterile unless the package is opened or damaged. The product is designed for single use. The method of sterilization is Gamma irradiation. Do not reuse or resterilize Contents: One (1) VASOVIEW 6 PRO Harvesting Cannula, One (1) 5 cc Syringe, One (1) Short Port Blunt Tip Trocar (BTT) with Endoscope Seal, One (1) Harvesting Cannula Seal, One (1) 30 cc Syringe, One (1) Dissection Tip. 2. Scope The essential requirement is for the contractor to provide an annual supply quantity of 60. Only the initial supply, quantity of 5, is to be delivered upon award. Orders for additional quantities up to the maximum annual quantity are to be placed as needed by authorized VASLCHCS Logistics staff. This request includes the option to increase quantities of items if additional needs above the annual estimate are identified. (5) Delivery: 10 Days ARO, FOB Destination, Deliver To: VA Health Care System Bldg 7 Warehouse 500 Foothill Drive Salt Lake City, UT 84148-0001 (6) Clause 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (7) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Product shall meet or exceed the specifications listed in the line item. Any substitution must provide detailed product descriptions and justification as to how the product deemed met or exceeded. No partial quote or shipment will be accepted. (8) Offeror shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. (9) Clause 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition.
