Description
Sources Sought The Department of Veterans Affairs, Veterans Integrated Service Network (VISN 8), James A. Haley Veterans Hospital, Tampa, FL is currently seeking firms, Service Disabled Veteran Owned Small Business, Veteran Owned Small Business, Hub zone, 8(A), Woman Owned Small Business OR Small Business with the ability to provide the items listed below. 1. Title of Project: Preventive Maintenance Inspection and Calibration of the Siemens PET/CT Scanner EE# 72726 site# 400-193845, WHB -509 2. Scope of Work: The contractor shall provide all resources (labor, materials/parts, tools, equipment, and transportation) necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. The vendor shall provide all parts, labor, and test equipment required to perform a Preventive Maintenance Inspection and System Calibration of the Siemens PET/CT Scanner EE# 72726 site # 400-193845 located in Building 56 3. Background: The Siemens PET/CT Scanner requires a Preventive Maintenance Inspection and Calibration to maintain Diagnostic Image Quality and Patient Safety 4. Performance Period: All work shall be performed Monday through Friday 7:00 AM to 5:00 PM. Work at the Government site shall not take place on Federal holidays unless directed by the Contracting Officer (CO). Reference Federal Holidays here or tell them where to find them. GENERAL REQUIREMENTS 1.The VA shall not provide service manuals or service diagnostic software to the Contractor. The Contractor shall obtain, have on file, and make available to its field service engineer (FSE), all operational and technical documentation (e.g., operational and service manuals, schematics, and parts lists) which are necessary to meet the performance requirements of this contract. The location and listing of the service data manuals, by name, and/or the manuals themselves shall be provided to the CO upon request. Any charges for parts, manuals, tools, or software required to successfully complete any service required are included within this contract and its agreed upon price unless specifically stated in writing otherwise. 2.The Contractor will provide all labor and replacement parts required to repair GE Innova 4100 vascular system to manufacturer's specifications and perform full system PM. All parts must meet or exceed manufacturer's factory specifications to maintain compatibility with systems presently in place and with future performance/reliability upgrades. Parts removed (replaced) by the Contractor become property of the Contractor. 3. At the conclusion of each scheduled maintenance visit, the Contractor will provide a written service report indicating the dates of service, the model, serial number, and barcode/ID number of the equipment serviced, the name of the service representative, the hours worked, description of the reported problem, and the services performed and parts replaced. The reports will be delivered to Contracting Officer Representative (COR) for signature when work is complete; a copy will be sent to Biomedical Engineering. During non-standard hours, reports will be taken to Police Dispatch for signature. 4. All removable media materials (laptop, DVD/CD, USB drive, etc.) will need to be scanned on site with approved anti-virus by OI&T or Biomedical staff and approved for use prior to the start of any work. CONTRACTOR EXPERIENCE REQUIREMENTS Each respondent must have an established business, with an office and full time staff to include a "fully qualified" field service engineer (FSE). The FSE(s) shall be authorized by the Contractor to perform the maintenance services. Respondents shall indicate their status as a foreign-owned or foreign-controlled firm and any contemplated use of foreign national employees on this requirement. The NAICS code for this sources sought is 811219, Other Electronic and Precision Equipment Repair and Maintenance. This has a size standard of $19 million. The government reserves the right to consider a small business set-aside based on responses to this sources sought. All information submitted by respondents to this sources sought synopsis is voluntary. This sources sought notice is not to be construed as a commitment by the Government, nor will the Government reimburse any costs associated with the submission of information in response to this notice. Respondents will not individually be notified of the results of any Government assessments. The Government's evaluation of the capability documentation received will factor into any possible forthcoming solicitation.
