- This is a Blanket Purchase Agreement opportunity with Adams & Associates, Inc./Gadsden Job Corps Center for Optical Services and not with the U. S. Department of Labor. Gadsden Job Corps Center - a 225 student residential training facility located at 600 Valley Street, Gadsden, Alabama is currently soliciting bids for Optical Services. All quotes should be submitted in writing to the Purchasing Department by Tuesday, June 24, 2014 by 3:00 pm. Quotes received after the deadline and/or without all requested documentation will not be considered. Vendor is to return written bid on company letterhead, vendor paperwork, all licenses for doing business in the State of Alabama, Certificate of Liability Insurance, Worker's Comp., Comprehensive Automobile Liability and a list of references, if no past experience with the Center. The period of performance is July 1, 2014 through November 30, 2014. Submitted bid will show a minimum breakdown as follows: *Cost per comprehensive Eye Exam *Cost per Metal of Plastic Frame *Cost per Pair of Plastic Lenses (Regular Prescription) *Printed Name and Signature of authorized person submitting bid Bids, Vendor Questionnaire, W-9, Authority to Commit and all licensures needed to do business may be faxed to 256-439-2980 or emailed to: long.cindy@jobcorps.org
