Classification Code: 6640 4. NAICS Code: 541690 5. Description: : High Resolution Computed Tomography Imaging of the Coronary Arteries and the Aorta This is a Small Business Sources Sought notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding: (1) the availability and capability of qualified small business sources; (2) whether they are small businesses; HUBZone small businesses; service-disabled, veteran-owned small businesses; 8(a) small businesses; veteran-owned small businesses; woman-owned small businesses; or small disadvantaged businesses; and (3) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. An organization that is not considered a small business under the applicable NAICS code should not submit a response to this notice. This notice is issued to help determine the availability of qualified companies technically capable of meeting the Government requirement and to determine the method of acquisition. It is not to be construed as a commitment by the Government to issue a solicitation or ultimately award a contract. Responses will not be considered as proposals or quotes. No award will be made as a result of this notice. The Government will NOT be responsible for any costs incurred by the respondents to this notice. This notice is strictly for research and information purposes only. Statement of Work (SOW) A. GENERAL INFORMATION 1. Title of Project: High Resolution Computed Tomography Imaging of the Coronary Arteries and the Aorta 2. Background Information: In spite of unprecedented advances in the diagnosis and management of atherosclerotic cardiovascular disease, it remains the leading cause of morbidity and mortality in the United States. This disease disproportionately affects older individuals and its incidence, prevalence, and severity all markedly increase with age. The VALIDATE study is a seminal research initiative designed by the intramural program at the National Institute on Aging (NIA) to address the critical question of why and how aging is associated with an increased prevalence of atherosclerotic disease. The study began 11 years ago and has successfully studied 498 participants, 399 of them were eligible for a second visit. Under the recently completed contract, 294 participants were planned to have their second visits done, however 27 were not due for the second visit within the time-frame of the contract due to later addition to the study, and a total of 266 participants were performed. Data from the 266 repeated studies did not provide statistical power to detect longitudinal changes in arterial parameters. Hence, the study investigators requested a contract to perform the second visits for coronary CT for the remaining 133 participants. This will complete the second visits for all eligible participants and provide full statistical power to examine the specific aims of the study. 3. Purpose or Objective of the Requirement: The objectives of this contract are to perform second visit high-resolutioncardiac CT of 133 participants first seen in VALIDATE I. This will allow the examination of the longitudinal changes in vascular properties in all VALIDATE participants who were eligible for a repeated visit. These imaging modalities will complement the other more traditional noninvasive cardiovascular tests performed in the VALIDATE study 4. Type of Contract Contemplated  Service contract, Open Market, 5. Period of Performance: One year B. CONTRACTOR REQUIREMENTS (SCOPE OF WORK) Independently, and not as agent of the Government, the Contractor shall provide all necessary personnel, facilities, supplies, and services to perform the work required by the contract as set forth below and in accordance with Attachment 2, Reporting Requirements. Specifically, the Contractor shall: 1. Arrange for scheduling and coordinating visits 2. Collect standardized history and physical on non-BLSA participants. 3. Perform high resolution, non-contrast CT imaging of the heart, coronary arteries, and proximal great vessels and provide Agatston calcium volume scores on the left main, left anterior descending, left circumflex, and right coronary arteries, and of the ascending and descending thoracic aorta in 79 BLSA participants (Group 1), 17 participants with "successful" vascular aging (Group 3), and 38 (Group 5) participants previously recruited to age-gender balance the first three groups. 4. Provide absolute values for Agatston calcium volume score as well as age-gender-ethnicity percentile ranking. These data should be provided in a format ready for merging with the core dataset of the VALIDATE study. 5. Provide official over-read reports of chest CT for incidental findings within 2-weeks of the study. C. GOVERNMENT RESPONSIBILITIES 1. Assist with scheduling imaging sessions for participants 2. Perform standardized history and physical on BLSA participants per the BLSA protocol. 3. Provide guidance in data management to assure consistency with data management methods followed at the NIA 4. Perform statistical analysis of data resulted and prepare manuscripts for publication D. DELIVERY OR DELIVERABLES Technical Reports The Contractor shall prepare and submit to the Contracting Officer's Representative (COR) Officer and Contracting Officer (CO) following items in the quantities and in accordance with the schedule specified in the contract. a) Report of Adverse Event- If during the performance of the tests or observation period which follows, a Baltimore Longitudinal Study on Aging (BLSA) participant experiences any untoward effects, the Contractor is to immediately contact the NIA COR at 410-350- 3989, or in his absence the Clinical Director at 410-558-8414. A written follow-up report documenting the event, Its outcome, and procedures employed is to be submitted electronically to the COR with a copy to the Clinical Director not later than 5 calendar days after the event. b) Test Results Report- The Contractor shall provide to the COR one electronic copy of the computed tomography (CT) test results not later than 30 calendar days after performance of an individual test. c) Semi-Annual Progress Reports- At semi-annual intervals, beginning not later than 15 days following the completion of 6 months of performance, the Contractor shall submit to the COR and to the Contracting Officer, one paper copy of the Semi-Annual Progress Report, including a summary of accomplishments to date and a listing of the results of the CT and the serologic tests performed during the preceding six months. The contractor shall also submit this report in electronic format. d) Final Report- Not later than one month after the contract completion date, the Contractor shall submit electronically, a final report to the COR and the Contracting Officer. The report shall contain the total number of tests performed and the results of these tests. e) Report of Salient Results - Included in the Final Report, to the Contracting Officer and the COR. E. TRAVEL None F. CONTRACTOR EXPERIENCE REQUIREMENTS - KEY PERSONNEL To assure consistency with CT studies already performed, contactor should meet the following requirements: 1. Personnel: readers should be Board Certified Cardiologists with longer than ten years of experience in interpreting coronary calcium CT scans and over than 500 scans performed. 2. CT scanner: Toshiba Aquilion One. 3. Scanning protocol: prospective ECG triggering protocol with a detector collimation of 3.0 mm x 4 slices at 74% of the RR interval, scanning from the carina to the diaphragm. G. SECURITY REQUIREMENTS: None H. DATA RIGHTS All data produced from this contract is property of the National Institute on Aging Intramural Research Program. I. SECTION 508 - ELECTRONIC AND INFORMATION TECHNOLOGY STANDARDS The contractor shall comply with Section 508 of the Rehabilitation Act (29 U.S.C. § 794d), as amended by the Workforce Investment Act of 1998 (P.L. 105-220), August 7, 1998. The COR is responsible for: (1) monitoring the Contractor's technical progress, including the surveillance and assessment of performance and recommending to the Contracting Officer changes in requirements; (2) interpreting the statement of work and any other technical performance requirements; (3) performing technical evaluation as required; (4) performing technical inspections and acceptances required by this contract; and (5) assisting in the resolution of technical problems encountered during performance. 3. <Include if applicable> [The alternate COR is responsible for carrying out the duties of the COR only in the event that the COR can no longer perform his/her duties as assigned.] 4. The Contracting Officer is the only person with authority to act as agent of the Government under this contract. Only the Contracting Officer has authority to: (1) direct or negotiate any changes in the statement of work; (2) modify or extend the period of performance; (3) change the delivery schedule; (4) authorize reimbursement to the Contractor for any costs incurred during the performance of this contract; (5) otherwise change any terms and conditions of this contract; or (6) sign written licensing agreements. Any signed agreement shall be incorporated by reference in Section K of the contract 5. The Government may unilaterally change the COR designation for this contract. Capability statement /information sought. Interested organizations must provide clear and convincing documentation of their capability of providing the item(s) specified in this notice. Interested organizations that believe they possess the ability to provide the required items must submit specific documentation of their ability to meet each of the project requirements to the Contract Specialist. Interested organizations must provide their Company Name, DUNS number, Physical Address, and Point of Contact Information. Interested organizations are required to identify their type of business, applicable North American Industry Classification System (NAICS) Code, and size standards in accordance with the Small Business Administration. The Government requests that no proprietary or confidential business data be submitted in a response to this notice. However, responses that indicate the information therein is proprietary will be properly safeguarded for Government use only. Capability statements must include the name and telephone number of a point of contact having authority and knowledge to discuss responses with Government representatives. Capability statements in response to this market survey that do not provide sufficient information for evaluation will be considered non-responsive. When submitting this information, please reference the solicitation notice number. The respondent must also provide their DUNS number, organization name, address, point of contact, and size and type of business (e.g., 8(a), HubZone, etc.) pursuant to the applicable NAICS code and any other information that may be helpful in developing or finalizing the acquisition requirements. One (1) copy of the response is required and must be in Microsoft Word or Adobe PDF format using 11-point or 12-point font, 8-1/2" x 11" paper size, with 1" top, bottom, left and right margins, and with single or double spacing. The information submitted must be must be in and outline format that addresses each of the elements of the project requirement and in the capability statement /information sought paragraphs stated herein.
