Description
The Naval Medical Logistics Command intends to award a sole source contract under the authority of FAR 13.106(b)(1) to Stereo Optical Company, Incorporated, 8623 W Bryn Mawr Avenue, Suite 502, Chicago, IL 60631, as the only responsible source that can provide Optec 900 Color Perception Testing Lanterns and Optec 2300 Armed Forces Vision Tester. Provisions and clauses in effect through Federal Acquisition Circular 2005-74 are incorporated. The U.S. Navy Fleet has a critical need for two (2) Optec 900 Farnsworth Lantern Color Perception Testing units for use during annual physical examinations and various screenings to determine hereditary color vision deficiency. Color perception test is a required exam in order to enter or to continue their eligibility for military service and special programs to include: Flying Duty Medical Examination (FDME), Bio-medical Equipment Technician (BMET), Submarine Duty, Naval Nuclear Propulsion, Nuclear Weapons Programs, Navy Diver and Special Operations Programs. The testing device shall be a U.S. Navy approved color perception testing unit capable of providing testing parameters to include, but not limited to, assessing color vision in aviation eye exam and other special program in the navy that require recognition of signal lights; light weight, high impact and water resistant; self-storing electrical cord with carrying case; scoring outline and test instructions; built-in electronic timer with override switch that is micro-processor controlled. Device shall be capable of operating on a 110-120VAC, 60Hz electrical supply. The U.S. Navy Fleet has a critical need for (1) Optec 2300 Armed Forces Vision Tester (AFVT) to be used in conducting visual acuity and depth perception requirements during annual physical examinations and various screenings to determine continued eligibility for military service and special programs to include: Aviation Duty, Submarine Duty, Naval Nuclear Propulsion, Nuclear Weapons Programs, Navy Diver and Special Operations Programs. The testing device shall be a U.S. Navy approved stereoscopic visual screening device capable of providing testing parameters to include, but not limited to, Monocular and Binocular Distant and Near Visual Acuity, Fusion and Depth Perception, Near Point Convergence, Oculomotor Balance, and Vertical and Lateral Phoria; shall include a scoring key for the operator to assess patient testing performance; include a forehead activator controlling the illumination of the testing fields; include a elevation adjustment to accommodate the differing heights of test subjects; incorporate a mechanism for changing the position of slides with an external indicator for the operator indicating the specific test slide visible to the test subject; incorporate an indicator visible to the operator designating FAR or NEAR test selection; incorporate an operator control panel with individual controls for right and left eye and Far/Near test selection. Device shall be capable of operating on a 110-120VAC, 60Hz electrical supply. Items being purchased in this acquisition: Qty 2 ea - Optec 900 Color Perception Testing Lanterns Qty 1 ea - Optec 2300 Armed Forces Vision Tester Qty 6 ea - Operator and Service Manuals Shipping - lot This acquisition is being conducted under simplified acquisition procedures. There are no set-aside restrictions for this requirement. The intended procurement will be classified under North American Industry Classification System 339112 with a Small Business Size Standard of 500 employees. This notice of intent is not a request for competitive proposals and no solicitation document exists for this requirement. However, parties interested in responding to this notice shall submit technical data, including cost, sufficient to determine capability in providing the same product. All capability statements received by closing of this notification of this synopsis will be considered by the Government. A determination by the Government not to compete based on responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. Based on the above information, the only technically acceptable contractor for providing Optec 900 Farnsworth Lantern Color Perception Testing units and Optec 2300 Armed Forces Vision Tester is Stereo Optical Company, Incorporated.
