Description

Request for Proposal- FDA-SOL-1126588 This is a combined synopsis/solicitation to obtain Access to Outpatient Prescription and Patient Level Drug Utilization Data utilizing the procedures set forth in FAR Parts 12 & 15. A detailed description of the required services is provided in Attachment 1 to the RFP. The solicitation number is FDA-SOL-1126588. All questions regarding the RFP shall be submitted to Ashley P. Allen via email at Ashley.Allen@fda.hhs.gov. The FDA must receive the questions no later than Thursday, June 6, 2014 at 2:00 p.m. ET. The subject line shall read: FDA-SOL-1126588, Access to Outpatient Prescription and Patient Level Drug Utilization Data. FDA will not answer questions received after this date and time. FDA will not answer questions asked via telephone or submitted to any official other than the Contract Specialist. Responses to questions will be posted on www.fbo.gov. The proposal shall be signed by an authorized official to bind your organization and must be received by the Contract Specialist no later than Friday, July 11, 2014 at 9:00 a.m. ET. Your proposal must be mailed, or hand delivered, to the following address: U.S. Food and Drug Administration (FDA) Office of Acquisitions and Grant Services (OAGS) Attn: Ashley P. Allen 5630 Fishers Lane, Room 1079 Rockville, MD 20857 Ashley.Allen@fda.hhs.gov Hand delivered proposal may be dropped off at the above location. However, please allow sufficient time as proposals cannot be accepted until they have gone through security. The FDA will not be held responsible for any delays that may be incurred getting your proposal through security.
