Description
Military Sealift Command Combined Synopsis and Solicitation 1. This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. 2. Solicitation number is MSC4064 and is being issued as a Request for Quotes (RFQ). 3. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-74, effective 30 May 2014. 4. The associated NAICS code is 541690 and this procurement is being set-aside for small businesses. 5. Military Sealift Command (MSC), Naval Base, Guam intends to award a firm fixed price purchase order for the following: Fall Protection Training The USS Frank Cable requires Fall Protection Training. Scope: The Contractor shall provide fall protection training to 48 personnel over the period of 18 Aug 2014 to 29 Aug 2014 during the hours of 0800-1600 (local time and date for Guam). a. Authorized Person or Authorized Rescuer Training that meets the requirements set forth herein as well as OSHA Requirements, ANSI/ASSE Z490 1-2009 and ANSI/ASSE Z359.2-2007 training conducted according to OSHA WCB, OH&S and Labor Code Requirements. Training shall consist of a total of twenty (20) hours per student to include an eight (8) hour hands-on rescue training component for each student during which the Gravitec G4 fall protection rescue kit shall be utilized. b. At the completion of training, a practical and written exam shall be administered. c. The Contractor shall provide a certificate of completion to personnel who successfully complete the course. The certificate shall state that the requirements addressed in paragraph a above have been met. The certificate shall be valid for a period of two years from course completion date. d. The Contractor shall provide a fall protection training manual for each student. e. The Contractor shall provide a comprehensive personal liability coverage for employee(s) conducting training. f. The Contractor shall furnish all required equipment to conduct the course. g. This order shall commence upon acceptance of the order and end when training and deliverables have been provided. 6. Place of performance: The training shall be provided shipboard at the following location: USS FRANK CABLE (AS 40) Polaris Point, Piti, Guam 96915 7. The provision at FAR 52.212-1, Instructions to Offerors - Commercial applies to this acquisition and is amended as follows: 52.212-1 (ADDENDUM) ADDITIONAL INSTRUCTIONS TO OFFERORS--COMMERCIAL ITEMS a. After receipt of quotes the Government may, with or without notice, negotiate with and, if desired, seek quote revisions from as many or as few quoters as it, in its discretion, deems appropriate. b. The term "offeror" or "offer" as used in FAR 52.212-1 shall be understood to mean "quoter" and "quote," respectively. Further, the term "award" shall be understood to describe the Government's issuance of an order. c. The following paragraphs in FAR 52.212-1 shall not apply to this RFQ: b, c, f (except f(4)), and l. d. The Government will consider all quotes that are timely received and may consider late quotes. Failure of a quote to address any items required in the submission package may make a quote unacceptable. e. SUBMISSION PACKAGE 1. The quote shall contain the following: a. Price quote which identifies the requested item(s), unit price, and extended price. b. Firm-fixed-Price total c. Remittance address, Tax identification Number, DUNS number and Cage Code 2. A completed copy of the representations and certifications at DFARS 252.212-7000 Representations and Certifications. 3. Page limitations for quotes are as follows: a. The quote, including the price, Technical Approach, but not including resumes, title page, table of contents, cover letter, completed representations and certifications, past performance information and exceptions to the RFQ, shall not exceed 5 pages in length. b. Pages beyond limits stated will not be evaluated. 8. Basis for the award: Award will be made to the responsible, technically acceptable quoter, whose quote, conforming to the combined synopsis/solicitation, offers the lowest evaluated price. 9. All quoters must include a completed copy of FAR 52.212-3, Offeror Representations and Certifications - Commercial Items with its offer. 10. The clause at FAR 52.212-4, Contract Terms and Conditions - Commercial Items applies to this acquisition. 11. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items applies to this acquisition. The following additional FAR clauses contained within FAR 52.212-5 are applicable to this acquisition: (a) The Contractor shall comply with FAR causes in this paragraph (b) that the contracting officer has indicated as being incorporated in this contract by reference to implement provisions of law or Executive orders applicable to acquisitions of commercial items. 12. This solicitation does not have a Defense Priorities and Allocation System (DPAS) rating. 13. Quotes may only be e-mailed directly to Maria.LeonGuerrero1@fe.navy.mil the sole designated e-mail address and inbox for receipt of e-mail submissions. No other electronic means of submission, used in whole or in combination with e-mail, is permitted. No other method of submission is acceptable. E-mail quotes shall be in either Adobe or Microsoft Office format. Quoters are advised that the Government may be unable to receive other types of electronic files (e.g.,.zip files) or files in excess of 20 megabytes. To be timely, an e-mail quote must be received in its entirety in the designated e-mail inbox by the due date and time for quote submission. Access to the MSC SSU Guam office is restricted. Emailed quotes may encounter unpredictable and lengthy routing delays. In all cases, quoters are responsible for the risks associated with the chosen delivery method and for ensuring the Government receives the complete quote at the appropriate designated location prior to the due date and time for quote submission. 14. The following additional FAR and DFARS clauses are applicable to this acquisition: FAR 52.212-2, Evaluation-Commercial Items applies to this request. Award will be based on lowest price technically acceptable. Award will be made without discussions; however, the Government reserves the right to open discussion anytime during evaluation. Technical acceptability means meeting the minimum specifications under the Statement of Work and providing the information required by FAR 52.212-1. All quotes will be ranked based on price. Only the lowest priced quote will be evaluated for technical acceptability, unless the lowest priced quote is deemed technically unacceptable. If the lowest quote is deemed technically unacceptable, the next lowest quote will be considered, and so on until an offeror is identified to be technically acceptable. a. Price: Quotes will be ranked first based on price from high to low. The price evaluation will document the realism, reasonableness, and completeness of the total evaluated price. b. Technical: The lowest offeror will move forward to the technical evaluation. If the lowest priced quote is deemed technically unacceptable, the Government will evaluate the technical acceptability of the next lowest offeror and so on until an awardee is identified. The award will be made to the lowest evaluated price whose offer is deemed technically acceptable. TECHNICAL CRITERIA RATINGS RATING DEFINITION Acceptable Quote clearly meets the minimum requirements of the solicitation. Unacceptable Quote does not clearly meet the minimum requirements of the solicitation. The provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items is included in this solicitation, and the offeror must include a completed copy of this provision with their quote or be currently registered in the SAM website with their Representations and Certifications completed.
