Description
To be honest and upfront we are submitting the following answers because they may be useful in helping you make decisions that your company may need to make when submitting a proposal. Use this information as a tool but be cautioned it is not an indicator of what the future workload may be. 1) The current contractor through the State of NH contract (not fed) fulfills the mission with the following personnel: Program Director. Program Coordinator. Clinical Director. Intake Coordinator. Financial Assistant Specialist. Office clerk (part time). Care Coordinators: 8 full time. 3 part time. Each Care Coordinator is given the expectation that they will work statewide, regardless of their physical location. Geography is one consideration given by the contractor in determining which Care Coordinator gets a case: however, it is low on the priority list. Higher priority is given to ensure that the Service Member and/or family is matched to a Care Coordinator who will most appropriately meet their needs through expertise, personality, and experience. 2) Role of Care Coordinator to Primary Care: There is no standard answer to this question. Confidentiality with the Soldier Member is very important: though if the Soldier Member requests that the Care Coordinator is involved with their Primary Care, then that is the expectation. 3) Previous RFI indicated that Resumes for Key Personnel are not required. That is not the case. Key Personnel Resumes ARE Required.
