Description
STATEMENT OF WORK Repair and Preventative Maintenance of American Medical Systems Green Light Laser VA New Jersey Healthcare System, East Orange Campus 1.Introduction The Facilities Management Service (138) at the VA New Jersey Healthcare System, East Orange Campus, seeks a service contractor to repair and perform preventative maintenance on an American Medical Systems Green Light Laser, Serial Number HPS2509, from 10/01/2014 to 09/30/2015. 2.Background The American Medical Systems Green Light Laser (Light), Serial Number HPS2509, is located in the Operating Room at the VA New Jersey Healthcare System, East Orange Campus. 3.Contractor's Technical Ability The contractor shall ensure the services required for the repair and preventative maintenance of the Light is performed in accordance with American Medical System Green Light Laser manufacturer's standards. 4.Payment for Work to be Performed Any work to be performed not covered under this Statement of Work (SOW) must be pre-approved by the Biomedical Engineer and/or designee. Additionally, a purchase order must be issued. The contractor's failure to obtain prior authorization will result in nonpayment. Payments are made in arrears. Payments will not be made for parts or services that are requested by any person other than the Biomedical Engineer and/or designee, or for any incomplete or incorrect invoices. 5.Specifications Servicing the Light includes but is not limited to cleaning, troubleshooting, lubricating, replacing worn or defective parts, calibrating, adjusting, and verification of the manufacturer's operational characteristics, specifications, and lubrications. All Light repairs and/or preventative maintenance inspections (PMI) shall be performed in accordance with the manufacturer's guidelines and specifications, and shall result in the Light operating in accordance with industry standard practices. Glass and flat detector are covered. 6.General Requirements Requests for technical support will be responded to within two (2) hours. The contractor shall be on site to perform repairs and preventative maintenance within eight (8) business hours of the time the service call was made by the Government. Repairs to the Light will be completed in accordance with the manufacturer's specifications within sixteen (16) business hours of the time of the service call was made by the Government. Major assemblies replaced shall be guaranteed against defect and/or failure for a period of one (1) year from the date of installation. All other components shall be guaranteed for a period of ninety (90) days. 7.Hours of Coverage All work shall be performed Monday through Friday, from 8:00 a.m. to 4:30 p.m., excluding Federal holidays and weekend. The Biomedical Engineer will only authorize overtime payment for emergencies and pre-approved situations. 8.Pre-Bid Inspections Prospective bidders are urged and expected to inspect the premises where services are to be performed to satisfy themselves regarding all conditions that may affect the cost of contract performance to the extent that the information is reasonably obtainable. Failure to do so will not relieve the prospective bidder from performing in accordance with the specifications defined herein and will be without additional cost to the Government. Appointments can be arranged through the Contracting Officer. 9.Preventative Maintenance Inspections There shall be two (2) preventative maintenance inspections for each system covered. The inspections shall be performed at a designated time between the months of October 2014 and September 2015. 10.Parts All parts shall be provided and replaced as necessary at no additional cost to the Government. Only new parts will be used routinely. Usage of refurbished parts will be subject to the approval of the Biomedical Engineer and/or designee. 11.Documentation The service representative is required to leave a service record with available Biomedical Shop personnel and obtain personnel's signature prior to leaving the premises. The service record will contain at least the following information: brief description of the purpose of the service call, work performed, part(s) used, and time spent for labor and travel. The contractor shall report to the Biomedical Shop to sign in upon arrival and sign out prior to leaving the premises. If the work is not completed prior to leaving the premises, a service record must be left, as per the above procedures, indicating the status of equipment. The Government will not render payment if the contactor fails to comply with the above procedures. 12.Special Requirements The contractor shall supply their own repair/maintenance manuals along with any other diagnostic software required to properly maintain the equipment in question. Service representatives are required to supply the Government with Material Safety Data Sheets (MSDSs) regarding any chemicals that are transported into the Medical Center prior to usage. The MSDSs are to be given to the Safety Officer. A set of MSDSs must also remain at the site where the chemical(s) is used. The contractor shall comply with usage of Personnel Protective Gear as defined in 29 CFR 19.10. The contractor shall also comply with all VA policies and regulations. 13.Contractor will Advise as to Qualifications The bidder shall show evidence of reliability, ability, and experience by furnishing a list of personnel who will perform under the contract, along with documentation of their competency, Manufacturer Certification, and type of experience. Names and addresses of other concerns and/or Government agencies for which comparable services are/were rendered shall be made available upon request. The ability to meet the requirements and the adequacy of information submitted will be considered by the Contracting Officer in determining the validity of the bidder. 14.Miscellaneous "Assemblies" are defined as a grouping of components, such a printed circuit boards, and/or electronic/mechanical devices which performs a sub-function within a system but are not necessarily stand alone in operation. "Components" include but are not limited to printed circuit boards, motors, relays, and keypads. 15.Period of Performance The Facilities Management Service (138) at the VA New Jersey Healthcare System, East Orange Campus, seeks a service contractor to repair and perform preventative maintenance on an American Medical Systems Green Light Laser, Serial Number HPS2509, from 10/01/2014 to 09/30/2015. The contractor shall complete the services under this Statement of Work (SOW) in _20_ calendar days or less from the date of award, unless otherwise directed by the Contracting Officer. If the contractor proposes an earlier completion date, and the Government accepts the contractor's proposal, the contractor's proposed completion date shall prevail. The work shall begin within _30_ calendar days of award, unless otherwise specified. Work at the Government site shall not take place on Federal holidays or weekends unless directed by the CO. 16.Place of Performance VA New Jersey Healthcare System, East Orange Campus 385 Tremont Avenue East Orange, NJ 07018
