The CBOC will house Primary Care and Mental Health functions as well as have Phlebotomy/Laboratory, Tele-retinal/Eye Exam, Audiology Exam and limited Pharmacy capabilities. The space plan was created with user input to optimize the space and functions. This building is due to be completed in August 2014. It is important that we have the CBOC up and running as close as possible behind building completion. Period and Place of Performance The installation of goods will take place at the CBOC located at: 1300 Veterans Drive Warrensburg, MO 64093 The estimated date for beginning installation and when product is needed onsite is November 3, 2013. Contract Requirements Required services are: 1.Single source of providing product/furnishings/medical equipment listed in this request as well as providing installation services for the product/furnishings/medical equipment. A single source of these items and installation will allow the awarded vendor to follow up with production and delivery of product as well as coordinate installation schedules. 2.As desired by the bidder, a pre-award site visit at designated time and day by the VA to be determined once solicitation is sent. 3.Provide all product specification options to Contracting Contact to be sent to Kansas City VA Medical Center Interior Design Program, where evaluation of quotes will be completed by the VA Interior Design. 4.Coordinate estimated arrival and installation times. 5.A post award site visit to perform to measure check of the facility confirming product fit. 6.Full installation services, including but not limited to artwork, countertops, systems furniture and wall mounted desks/computer arm tracks. 7.Submittals provided as specified. Each member of the installation team will need to provide a valid ID to the VA Interior Design prior to attending the safety course. This will give the installation crew the necessary current badge they must wear to perform the work in our medical facility. The installation will have to be scheduled with the KCVA Interior Design Office based on the time that the clinical team believes will be least disruptive to our patients.
