- Attachment C - Past and Present Performance Survey Attachment B - Past Performance Profile U.S. Department of State Statement of Work U.S. Department of State Clinical Laboratory Services This is a combined synopsis/solicitation under Solicitation Number SAQMMA14Q0049. The awarded contract shall provide the Department of State’s Office of Medical Services (MED) with clinical laboratory testing services. MED performs physical exams to determine availability of Foreign Service Officers and their families of assignment overseas to approximately 200 embassies and consulates. As such, the Office of Medical Services (MED) performs 6500 to 7000 physical examinations per year that require laboratory tests and results. Although most testing is done in house by the Office of Medical Services’ Laboratory Department, some of these tests much be sent out for testing. MED also receives samples from State Department overseas activities for testing. Furthermore, our overseas posts will be required to send some time sensitive tests directly to the testing laboratory to maintain the viability of the sample. The award contract will include a base year and four one year option periods.
