Description: DURABLE MEDICAL EQUIPMENT (DME) SERVICES for multiple VISN 15 VA Medical Centers Request for Quotation (RFQ) - in accordance with FAR part 12 (Acquisition of Commercial Items) and FAR Subpart 13.5 (Test Program for Certain Commercial Items), Network Contracting Office (NCO) 15 is soliciting quotations for furnishing all equipment, labor, transportation and supplies required to provide delivery, pickup, setup removal, storage and repair services for government owned Durable Medical Equipment (DME) Services for VA Medical Center Beneficiaries within one or more specific Veteran Integrated Services Network (VISN) 15's eight (8) Medical Center service areas. Per the attached RFQ #VA255-14-Q-1123 Statement of Work (SOW), terms and conditions. -The procurement will be a 100% Small Business Set-Aside. The NAICS code for this requirement will be 621610 with a size standard of $14 million. -Quotations will be evaluated on Technical Excellence, Past Performance and Price in accordance with the evaluation criteria identified in the solicitation. -Award will be Best Value by facility or group of facilities. -Offerors may provide a quote for one, more than one or all of the facilities identified in the Statement of Work (SOW). -The Government anticipates awarding a one (1) year contract with four (4) one (1) year option periods.
