Description

Solicitation VA248-14-Q-1220 The purpose of this Amendment (#3) is to correct the statement of work, paragraphs 1.4 and 1.11. Paragraphs 1.4 and 1.11 and evaluation factor 1. The following are corrected to now read: Page 4 of 59, para 1.4 (statement of Work) 1.4.Period of Performance: This is a one-time job. The period of performance shall not exceed three weeks after award notification. Page 5 of 59, para 1.11 (Statement of Work) 1.11.Key Personnel (Project Manager/Alternate Manager): The contractor shall provide one project manager who shall be responsible for the overall performance of the work. The contract manager or alternate shall have full authority to act for the contractor on all contract matters relating to of this contract. The contract manager or alternate shall be available during hours of operation as stated herein or approved by COR. Additionally, PM is required to be on call and ready to respond to matters concerning OVAMC Lake Nona hyper-chlorination. The Project Manager shall have minimum of (5) years of management experience in related field (water treatment). Page 55 of 59 (52.212-2 -- Evaluation -- Commercial Items) 2.Basis of award. Subject to the provisions contained herein the Government intends to make a single, Firm Fixed-Price, award resultant from the issuance of this solicitation that present the best value to the government. Award will be made to the responsible offeror whose offer conforming to the statement of work and request for quote represents the lowest price technically acceptable offer to the Government, price and other factors considered. The following factors shall be used to evaluate offers: 1)Technical Capability/Approach/Management Technical Capability/Approach (as outlined in SOW/Part 2 Specific Tasks) Delivery Schedule (as outlined in SOW/para 1.4) PM Resume OSHA Certification Certification (State of Florida Certified/licensed General Contractor) 2)Past Performance 3)Price Submit offers by solicitation close date July 10, 2014 Time: 1:00 PM (EST) to rosa.rivera3@va.gov.
