- Request for Information (RFI) Project Title: Radiological Interpretation Services for New Mexico Veterans Affairs Health Care System (NMVAHCS) Project Number:VA258-14-R-0504 RFI Number:VA2585-14-R-0501_1 Synopsis This is a Request for Information (RFI) as defined in FAR 15.201(e). The Government is requesting information regarding the availability and feasibility of attracting new Contractors to meet our need defined below. This request is to gather information from interested parties. Only interested parties shall review the information below and reply no later than July 16, 2014. Statement of Need The Contractor shall provide the services of qualified, experienced and board certified/board eligible Radiologist to perform interpretations of radiological images, teaching of residents and consultation services on site in accordance with the specifications contained herein to beneficiaries of the Department of Veterans Affairs and Air Force eligible by virtue of Joint Venture Sharing Agreements in accordance with all terms, conditions and provisions. NMVAHCS Radiology Services are accredited by American College of Radiology (ACR) and follow the standards & guidelines set forth by ACR. The Contractor providing onsite radiological interpretation services shall provide services that meet or exceed the current ACR Guidelines. Background The NMVAHCS is a Level 1 tertiary referral center with a 24-hour Emergency Room located in Albuquerque, NM. The NMVAHCS is authorized to operate 310 beds, which includes 184 acute hospital beds and a 26 bed Spinal Cord Injury Center. The Imaging Department provides a full range of essential services utilized in completing the daily task of providing superior care to Veterans. Currently, the NMVAHCS performs 2100 Radiological exams a week. Qualifications Physician fee payment for the interpretation of diagnostic radiology and other diagnostic tests will be made using Current Procedural Terminology (CPT) codes established by the American Medical Association (AMA). The CPT codes will be updated yearly and the most current edition of the AMA CPT Manual and associated Medicare and Medicaid rates will be used in determining the pricing schedule. Centers for Medicare and Medicaid Services (CMS) Guidelines shall be adhered to regarding physician participation and/or supervision necessary to receive payment for procedures and examinations furnished by a resident within a teaching setting. Title 42 Code of Federal Regulations (CFR) 415.180 is referenced as a condition of payment: § 415.180 Teaching setting requirements for the interpretation of diagnostic radiology and other diagnostic tests. (a) General rule. Physician fee schedule payment is made for the interpretation of diagnostic radiology and other diagnostic tests if the interpretation is performed or reviewed by a physician other than a resident. (b) Documentation. Documentation must indicate that the physician personally performed the interpretation or reviewed the resident's interpretation with the resident. Procedures performed by the Contractor during radiological interpretations and other specialty diagnostic procedures will be fully documented, verified and reimbursed at the negotiated rate. These procedures and their subsequent CPT code(s) shall be subject to Government review and acceptance/rejection shall be based upon the medical necessity of such procedure(s). Determinations of medically necessary care are as determined by a physician exercising clinically prudent judgment in accordance with generally accepted standards of medical practice and less costly alternatives are permissible only when they are at least as likely to produce equivalent diagnostic results. The per-procedure Radiological Interpretation Services shall include the following: a.Education and supervision of Radiology residents will be provided in accordance with the Medical Education Affiliation Agreement (VA FORM 10-0094a) and MCM 14.1 b.Coverage for Resident conference/training requirements such as Noon call responsibilities will be shared between VA and Contractor radiologists on a mutually agreed basis. Noon call stat reads may be handled electronically through the VA interconnection with the Contractor. c.Vacation and meeting coverage for VA radiologists will be provided with 4 weeks' notice. This will be coordinated through the COR and will be in writing. d.Tumor Board attendance will be shared between VA and UNM radiologists on a mutually agreed basis. This will be in writing and coordinated through the Chief of Radiology of the NMVAHCS. The exams to be interpreted include but are not limited to: General Radiography, Magnetic Resonance Imaging (MRI), Nuclear Medicine, Positron Emission Tomography (PET), Computed Tomography (CT), Ultrasound (US), Mammography, Guided Breast Biopsies, Fluoroscopy and DEXA scans. The Contractor shall provide a sufficient workforce to interpret approximately 79,431 radiological images annually. Each interpretation shall specify the images reviewed, findings and impression, VA diagnostic code appropriate to the impression, communication with referring clinicians and when appropriate, recommendations for management. Training to residents shall be provided for each reading of radiological images. In the case of unexpected or urgent significant abnormalities detected on examinations, the Radiologist shall contact the referring NMVAHCS clinician directly regarding the findings and document this communication in their report. Any erroneous interpretations shall be amended promptly by the Radiologist upon discovery of the error(s) at no additional cost. All significant errors (i.e., those that could reasonably be expected to change the patient's management) shall be reported promptly by the Radiologist to the referring clinician as well as the Chief of Radiology. The communications shall be documented by the Radiologist in his/her amended report. The Contractor shall provide teaching and professional guidance to residents and fellows supervision in compliance with Veterans Health Administration (VHA) resident handbook procedures. If VHA Handbook 1400.1 is updated, copies of current handbook can be obtained from the NMVAHCS's Department of Radiology. Radiology and Nuclear Medicine procedures are to be performed either by the attending physician or with the attending physician providing professional direction to the house staff at levels defined in VHA Handbook 1400.01, entitled Resident Supervision published December 19, 2012. Appropriate staffing to address the daily work load as well as supervise Residents shall be the responsibility of the Contractor. Assignments for coverage at the NMVAHCS will be coordinated by the Chief of the Imaging Service and a Contractor designee. Contract physician(s) assigned by the Contractor to perform the services covered by this contract shall have a current license to practice medicine in any State, Territory, or Commonwealth of the United States or the District of Columbia. All licenses held by the personnel working on this contract shall be full and unrestricted licenses. Contract physician(s) who have current, full and unrestricted licenses in one or more states, but who have, or ever had, a license restricted, suspended, revoked, voluntarily revoked, voluntarily surrendered pending action or denied upon application will not be considered for the purposes of this contract. All contract physician(s) shall be board certified. All continuing education courses required for maintaining certification must be kept up to date at all times. Documentation verifying current certification shall be provided by the Contractor to the NMVAHCS COR on an annual basis for each year of contract performance. Credentialing and privileging shall be done in accordance with the provisions of VHA Handbook 1100.19 referenced above. The Contractor is responsible to ensure that proposed physician(s) possesses the requisite credentials enabling the granting of privileges. No services shall be provided by any contract physician(s) prior to obtaining approval by the NMVAHCS Professional Standards Board, Medical Executive Board and Medical Center Director. If a contract physician(s) is not credentialed and privileged or has credentials/privileges suspended or revoked, the Contractor shall furnish an acceptable substitute without any additional cost to the government. Contract physician(s) shall be technically proficient in the skills necessary to fulfill the government's requirements, including the ability to speak, understand, read and write English fluently. Contractor shall provide documents upon request of the CO/COR to verify current and ongoing competency, skills, certification and/or licensure related to the provision of care, treatment and/or services performed. Contractor shall provide verifiable evidence of all educational and training experiences including any gaps in educational history for all contract physician(s) and contract physician (s) shall be responsible for abiding by the Facility's Medical Staff By-Laws, rules, and regulations that govern medical staff behavior. National Provider Identifier (NPI) is a standard, unique 10-digit numeric identifier required by HIPAA. The Veterans Health Administration must use NPIs in all HIPAA-standard electronic transactions for individual (health care practitioners) and organizational entities (medical centers). The Contractor shall have or obtain appropriate NPI and if pertinent the Taxonomy Code confirmation notice issued by the Centers for Medicare and Medicaid Services (CMS) National Plan and Provider Enumeration System (NPPES) be provided to the Contracting Officer with the proposal. The Contractor and all contract physician(s) are responsible for identifying and communicating to the CO and COR conflicts of interest at the time of proposal and during the entirety of contract performance. At the time of proposal, the Contractor shall provide a statement which describes, in a concise manner, all relevant facts concerning any past, present, or currently planned interest (financial, contractual, organizational, or otherwise) or actual or potential organizational conflicts of interest relating to the services to be provided. The Contractor shall also provide statements containing the same information for any identified consultants or subcontractors who shall provide services. The Contractor must also provide relevant facts that show how it's organizational and/or management system or other actions would avoid or mitigate any actual or potential organizational conflicts of interest. These statements shall be in response to the VAAR provision 852.209-70 Organizational Conflicts of Interest (Jan 2008) and fully outlined in response to the subject attachment in Section D of the solicitation document. The Contractor shall provide interpretations of Routine, Urgent and STAT examinations during the hours of 0730 to 1600, Monday through Friday. The Contractor shall provide interpretations of STAT examinations during the hours of 1600 TO 0730, Monday through Friday, weekends and all Federal Holidays. The Contractor shall provide a network of highly experienced, Board Certified Radiologists with a spectrum of sub specialties experience and advanced HIPAA compliant technology experience. Physician shall dictate findings, review, electronically sign and upload reports Into the AVAHCS's Computerized Patient Record System (CPRS). Contract physician(s) providing healthcare services to the NMVAHCS patients shall be considered as part of the Department Healthcare Activity and shall comply with the U.S.C.551a (Privacy Act), 38 U.S.C. 5701 (Confidentiality of claimants records), 5 U.S.C. 552 (FOIA), 38 U.S.C. 5705 (Confidentiality of Medical Quality Assurance Records) 38 U.S.C. 7332 (Confidentiality of certain medical records), Title 5 U.S.C. § 522a (Records Maintained on Individuals) as well as 45 C.F.R. Parts 160, 162, and 164 (HIPAA). Physician services shall be performed in accordance with the policies and procedures of NMVAHCS and its medical staff by-laws and service under the direction of the Chief, Imaging Service or designee. Contractors are required to be registered in the System for Award Management (SAM) to be considered responsive and capable. This is not a solicitation. This RFI is issued solely for information and planning purposes. All information received in response to this RFI will be treated as business proprietary data if so marked and exempt from disclosure under the Freedom of Information Act. Nonetheless, responders should properly mark their responses if the information is proprietary. Responses to the RFI will not be returned. In accordance with FAR 15.201(e), responses to this notice are not offers and shall not be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI.
