Attachment 4: Wage Determination 05-2103 Attachment 3: Solicitation Instructions, Provisions and Clauses Attachment 2: Price Quotation Matrix Attachment 1 - Statement of Work I. SOLICITATION INFORMATION This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This simplified acquisition will be conducted pursuant to FAR Subpart 13.106 procedures. The National Transportation Safety Board (NTSB) RFQ solicitation number is: NTSBQ140031, and is issued as a request for quotation (RFQ). This solicitation document incorporates all mandatory commercial item provisions and clauses are those in effect through Federal Acquisition Circular 05-74; effective July 31, 2014. The associated North American Industrial Classification System (NAICS) code is 561622 - Locksmith Services and Product Service Code of category S216: Housekeeping - Facilities Operations Support. The small business size standard is $19.0 million. This requirement is being solicited as local Washington Metropolitan Area, 100% total small business set-aside. II. REQUIREMENTS The NTSB requires commercial locksmith services for the headquarters facility located at 490 L'Enfant Plaza SW, Washington, DC 20594 for its offices occupying three floors, and the NTSB Boardroom and Conference Center located at 429 L'Enfant Plaza SW, Washington, DC 20024. The Offeror shall provide the following requirements, in its entirety, in accordance with the Attachment 1 - Statement of Work (SOW). The place of performance is Government's facility. The period of performance is comprised of a base year (12-months) and four (4) option years. The base year requires rekeying of the 4th floor of approximately 100 office doors. In addition, ad hoc locksmith services are required for the base and four (4) option years. The contract line items (CLINs) are enclosed in Attachment 2: Price Quotation Matrix. III. SOLICITATION INFORMATION & BASIS OF AWARD 1. Offerors must submit technical and pricing requirements as set forth in Attachment 3, Section I - Solicitation Instructions. 2. The Basis for Award will be made to the lowest priced, technically acceptable quotation. To receive consideration for award, a rating of Acceptable must be achieved for the Technical Factors. Offerors are advised that in the interest of efficiency, the Government reserves the right to conduct the evaluation in the most effective manner. Specifically, the Government may first evaluate the total quoted price (base and four option years) of all Offerors resulting in the evaluation of the technical proposal, of the lowest priced Offeror only. If the lowest priced Offeror's technical proposal is determined to be rated as Acceptable, the Government may affirm award to that Offeror without further evaluation of the remaining Offerors' technical proposals. If the lowest priced Offeror's technical quotation is determined to be rated as Unacceptable, then the Government may evaluate the next lowest priced technical quotation, and so forth, until the Government reaches the lowest priced technical quotation that is determined to be rated as Technically Acceptable. Quotation Submission Deadline: Quotation submissions will only be accepted in electronic form to Wendy Kwong, Contract Specialist at email Wendy.Kwong@ntsb.gov. The subject line of the email must contain the solicitation number and the "Locksmith Services" description. IMPORTANT NOTE: The maximum size of an email, including attachments, which the NTSB email server can accept is 15 megabytes (MB). Therefore, the offeror may choose to send their quote in separate emails, if the combined file size of any email received by the NTSB email server will exceed 15 MB. It is the offeror's responsibility to ensure that the designated point of contact has received your quote at the email address of the contact specified above by the quote due date. Quotations must be received by Wednesday, July 23, 2014 NLT 2:00 PM/EST. Any responses received after the closing times and dates listed above will not be considered.
