Description
FACTOR 1 Technical Capability: The Technical Capability evaluation is divided into three (3) sub-factors. Sub-factors 1 thru 3 are of equal importance. The technical rating will be based on the extent to which the offeror demonstrates the following: 1. License/Credentials/Services: Provide licenses/credentials/certification of all personnel who will perform under this contract (names, qualifications, position title). Must be Board Certified/Board Eligible Gynecologists. The Gynecologists provided must be practicing Gynecologists experienced in surgical management of the entire scope of the clinical pathology involving female pelvic floor and reproductive organs. 2.Quality Assurance Program: Offeror to provide a written Performance Quality Assurance Plan describing the offeror's ability to meet and assure performance in accordance with the terms and conditions of the solicitation. 3.Gynecologists must have at least two (2) years of recent related experience. FACTOR 2 Past Performance: An offeror's past performance is important in determining its ability to successfully perform required services. The government is especially interested in recent and relevant past performance that relates very closely to the requirement set forth in the performance-based work statement. FACTOR 3 SDVOSB Participation: The mission of the Department of Veteran Affairs (VA) is "To care for him who shall have borne the battle and for his widow and his orphan." These words, spoken by Abraham Lincoln, are focus of VA's endeavors to serve our Nation's veterans and their families. Buying the goods and services necessary to operate the Department from Veteran-Owned Small Businesses (VOSB) and Service-Disabled Veteran-Owned Small Businesses (SDVOSB) is in direct support of this mission. In accordance with Veterans Affair Acquisition Regulation (VAAR) 852.215-80, the VA shall evaluate offerors based on their service-disabled veteran-owned or veteran-owned small business status and their proposed use of eligible service-disabled veteran-owned small businesses and veteran-owned small businesses as subcontractors. Eligible service-disabled veteran-owned offerors will receive full credit, and offerors qualifying as veteran-owned small businesses will receive partial credit for the Service-Disabled Veteran-Owned and Veteran-Owned small Business Status evaluation factor. To receive credit, an offeror must be registered and verified in Vendor Information Pages (VIP) database. (http://www.VetBiz.gov). Non-veteran offerors proposing to use service-disabled veteran-owned small businesses or veteran-owned small businesses as subcontractors will receive some consideration under this evaluation factor. Offerors must state in their proposals the names of the SDVOSBs and VOSBs with whom they intend to subcontract and provide a brief description of the proposed subcontracts and the approximate dollar values of the proposed subcontracts. In addition, the proposed subcontractors must be registered and verified in the VetBiz.gov VIP database (http://www.vetbiz.gov). FACTOR 4 Price/Cost: 1. Technical and past performance, when combined, are significantly more important than price.
