Description

The Bath VA Medical Center located at 76 Veterans Avenue, Bath, NY has a requirement for a service maintenance agreement for one Mallinckrodt OptiVantage DH Media Injector system. The service maintenance plan shall include 24hr/7day a week telephone support, on-site support with a response time of 8hrs from receipt of initial call (Monday to Friday 8am to 4:30pm), one scheduled preventative maintenance service in accordance with manufacturer's instructions. The NAICS Code is 811219. The expected cost range for this firm fixed price contract to include two option years is less than $25,000.00. All responsible sources may submit a capability statement, proposal, or quotation which shall be considered by the three VA Medical Centers. This is not a solicitation.
