Department of Veterans Affairs Date: July 14, 2014 From: Office Acquisition and Logistics, National Acquisition Center, Business Resource Service Solicitation Number: VA797T-14-P-0089 Notice Type: Combined Synopsis/Solicitation This is a combined synopsis/solicitation for commercial items prepared IAW the format in 12.6, as supplemented with additional information included on this notice. This announcements constitutes the only solicitation; all responsible sources may submit capability statement - a written solicitation will not be issued. Sole Source Procurement. The Government has determined that this is a sole source procurement. This determination is based on strict specifications for the use of OEM components and adherence to OEM's repair specifications by the manufacturer, Medivators of the sterilization system- Automated Endoscope Reprocessors (AER's) (utilized at the Miami VA Healthcare Center) and the Original Equipment Manufacturer (OEM) Karl Storz Endoscopy - America, Inc. The use of OEM maintenance and repair service and parts are essential to the sterilization validation of medical devices to ensure patient's safety. The Government intends to solicit and negotiate with only one source, Karl Storz Endoscopy - America, Inc. and issue a firm-fixed-price purchase orders on a sole source basis with the OEM. All responses will be reviewed by the agency for the purpose of determining whether to conduct a competitive procurement for this and future requirements Responses must include technical specifications of services, demonstrating ability to provide maintenance and repair services of described equipment and reprocessing validations for the described AER. A determination not to compete this and future requirement, based upon responses to this notice, is solely within the discretion of the Government. The Government will not be responsible for any cost incurred by responding to this notice. All quotes/responses shall be submitted by email to, mechelle.reaser@va.gov no later than Thursday, July 17, 2014, 1:00 PM CT. Please contact, Mechelle D. The requirement is as followed: Maintenance and repair service of Karl Storz endoscopes, models, Flexible Cystoscope, model number (No.): 11272CU1, Video Bronchoscope, model No. : 11900BN, and one Flexible Rhino - Laryngoscope, model No: 11101RP2. 0001: Flexible Cystoscope, model No.:11272CU1 - Maintenance and repair services include, but not limited to angle cover cut / leak, braided area bunched, void around the distal light guides. Quantity: 1 0002: Flexible Rhino-Laryngoscope, model No.: 11101RP - Maintenance and repair services include but not to crushed in vertebrae system, over bend/crushed in vertebrae system (shaft), angle cover leak, broke image Quantity: 1 0003: Flexible Cystoscope, model No.:11272CU1 - Maintenance and repair services include but not limited to angle cover cut / leak, vertebrae soiled /corrosion Quantity: 1 0004: Flexible Cystoscope, model No.:11272CU1 - Maintenance and repair services include but not limited to angle cover cut / leak, missing distal window, void distal light guide Quantity: 1 0005: Flexible Cystoscope, model No.:11272CU1 - Maintenance and repair services include but not limited to angle cover cut / leak, channel - thermal laser damage, distal window- void in Epoxy, light fiber tip, vertebrae soiled/corrosion.
