- SOURCES SOUGHT ANNOUNCEMENT: The Department of Veterans Affairs Lebanon VA Medical Center, 1700 South Lincoln Avenue, Lebanon, PA 17042 is conducting a market survey to identify potential sources to 864 boxes (50per box) Roche Diagnostic ACCU-CHEK Inform II Test Strips for market research purposes only. Sources are invited to demonstrate their capabilities to supply 864 boxes (50per box) Roche Diagnostic ACCU-CHEK Inform II Test Strips. Sources are asked to provide information that demonstrates their answers to the following information: 1.Company Name, Address, Website, POC, and POC Phone Number. 2.The North American Industry Classification System Code (NAICS) is 325413 and the small business size standard is 500 employees for this type of effort. Based on this information, please indicate whether your company would be a Large or Small Business. 3.If company holds a Federal Supply Schedule (FSS) Contract, please list the GSA Contract Number and relevant SINS, and affirmative statement that these items are available on your GSA schedule.
