*****Begin Text Document- 'FBO-DESCRIPTION' ***** This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement is being issued as a request for proposal (RFP) and constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular #2005-71 Effective 25Nov2013. This combined synopsis/solicitation is being competed on a 100% Service Disabled Veteran Owned Small Business (SDVOSB) set aside. This acquisition will be competed to the full extent required by regulation. The NAICS code that applies is 562910 and business size is $14.0 Million. This requirement is for: Removal and disposal of approximately 550 square feet of 12" x 12" vinyl floor tile and ACM adhesive. Location is Rooms LA135B, LA136A, LA140, LA140A and LA144 in accordance with South Carolina DHEC, EPA and OSHA regulations. VA supplies one (1) 208 volts, 3-Phase 50 amp circuit and connects the vendor-supplied pigtail plug to our circuit to power the vendors heat machine. Quote submitted must at a minimum Meet or Exceed Specification for services requested. Each quote will be evaluated in accordance with the Meet or Exceed Standard. Note: Please review the solicitation document in its entirety and the Statement of work for detail instruction on what is required prior to submitting your proposal and how it will be evaluated. Failure to provide require documentations (in accordance with SOW and Evaluation Factors), your proposal will be considered none responsive and will not be considered for evaluation or award. This is a Lowest Price Technically Acceptable (LPTA) acquisition. The Government will make an award to the responsible offeror whose offer conforming to the solicitation is the most advantageous to the government.
