Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Solicitation Number is W912KN-14-T-0023 Request for Quotation. The solicitation incorporates provisions and clauses 52.212-2, 52.212-3, 52.212-4, 52.212-5, 252.212-7001, 252.225-7001. (NAICS) code is 621511. This requirement is set-aside for small business. The size standard is $13.5. The Standard Industrial Code (SIC) is 8000. The Federal Service Code is Q301. The NJARNG plans to award a Centralized Blanket Purchase Agreement (BPA) contract with a base period of 1 Oct 2014 through 30 Sep 2015 and four 1-year options. The total possible duration of the contract would not exceed five years. The contractor is required to provide a pricing plan for the five years. Description of Agreement: This Blanket Agreement (BPA) is a written instrument of understanding between your firm and the Contracting Officer, Army National Guard, USPFO for New Jersey. This agreement pertains to the acquisition of personal services to provide necessary labor and materials for laboratory blood testing results in support of the National Guards physical examination requirements as requested by the Contracting Officer or his/her designated representative. Periodic physical exams and Soldier Readiness Processing (SRP) require specific laboratory testing for each soldier. Specifically, every soldier requires G6PD testing. A minimum of 0 and a maximum of 1200 laboratory studies shall be given per month within a one year period. There is no guaranteed minimum number of tests. Most or the testing will be done on weekends. Specific laboratory studies required are: Serum Cholesterol (For anyone 38 years old or under), Serum fasting glucose (for anyone 40 years old and above), High density lipoprotein (HDL) (For anyone 40 years old and above), Low density lipoprotein (LDL) (For anyone 40 years old and above), PSA (Over 40, Physicians Request Required), Trigly(Triglycerides), FBS, and CBC w/differential. G6PD A minimum 0 and maximum of 5 quantities on each test on a given weekend within a one year time period. Any of these tests can be given at anytime for any given age. Serology, Chemistry and Hematology. (Misc other tests from these categories) The contractor shall provide all personnel, equipment, tools, materials, supervision, and other items and services necessary to perform medical laboratory services as defined in this Performance Work Statement (PWS). The selected firm must provide, as part of the agreement, a courier service to the National Guard Training Center, Sea Girt for both routine and STAT specimen transport to the laboratory. The contractor/laboratory must pick-up within 24 hrs and provide test results within 24hrs after receipt of the blood specimens. The contractor shall provide specific results for the required blood tests. Retrieval of lab results must be available through the internet or fax. Abnormal test results will be prominently marked. For each lab value, the range of normal used by the Contractors lab will be indicated. The form to be used for recording the laboratory blood results must contain at a minimum the following information that will be provided by the NJARNG: Patients name, Patients social security number, Our billing address, Date blood was drawn, Time blood was drawn and Ordering Physician. Medical record maintenance must conform to all OSHA and NIOSH requirements in terms of content and record keeping duration. All records remain the property of the New Jersey Army National Guard (NJARNG) Medical Facility. Upon written request, the Contractor will provide all records to the Government within 7 working days. Individuals authorized to order services from this contract shall be designated in writing by the Contracting Officer and will be provided to the contractor. The Contracting Officer shall provide to the contractor at the time of the award the name, rank, phone number, fax number, e-mail address of each individual that is authorized to place work orders against this contract. This delegation shall be in writing and shall include the signatures of the individual and the Contracting Officer. The contractor shall be required to acknowledge receipt of this certification and that they understand that only individuals delegated in writing shall be authorized to place work orders. The Contracting Officer shall notify the Contractor in writing of any changes in regards to individuals authorized to place orders against this contract. A designated person will notify the contractor of the estimated quantity for each test required by telephone approximately 3 days prior to the testing. The oral order will be followed by submission of the Lab Test Request Form indicating the exact number of each request required. Contractor shall provide forms and collection tubes. Laboratory Certification: The contractor must ensure all clinical laboratory/chemistry services are performed by a laboratory licensed by the US Department of Health and Human Service Health Care Financing Administration pursuant to the terms of the most current Clinical Laboratories Improvement Act with amendments (42 U.S.C. 263a) and accredited by the College of American Pathologists. Operation of these services subjects the facility to Clinical Laboratories Improvement Amendment (CLIA) certification every 2 years. Supervisors of laboratory services are required to possess a baccalaureate degree in the laboratory services in order to meet CLIA requirements. Clinical laboratory/chemistry services are to be performed in accordance with 29 CFR 1910.1030 Blood borne Pathogen Standards. All Testing must be conducted by qualified occupational health personnel. These are medical personnel, such as physicians, nurses, etc., who by virtue of educational training and experience have acquired competence in Industrial Hygiene and Occupational Health, and who are familiar with and, who can conform to the standards set by the national Institute of Occupational, Safety Health (NIOSH). No blood testing will be scheduled on Federal Holidays. When a holiday falls on a Sunday, the following Monday will be observed as a holiday. When a holiday falls on a Saturday, the preceding Friday is observed as a holiday. The following holidays are observed: January 1st 3rd Monday of January 3rd Monday of February Last Monday of May July 5th 1st Monday of September 2nd Monday of October November 11th November 25th December 24th December 31st Billing will indicate the following: Contractor name and address, contract number, work order number(s), itemized listing of services performed include the names of all employees tested, quantity and unit price and extension of each item. Selection will be based on the following evaluation factors, in order of importance: (1) total cost or price. The Government will evaluate offers for award purposes by adding the total combined price of one each test listed above for an aggregate price. The aggregate price for the base year and all options years will be combined into an overall price. Low price evaluation of options will not obligate the Government to exercise the options. (2) past performance, caliber of offerors performance on previous contracts of a similar nature. INSTRUCTIONS: Offers shall include: (1) a quote for the aggregate combined price of one each of all tests listed above for total price for the base year and a total aggregate price for each option year; (2) a price list showing the base year price of each test and the price of each test projected for each option year; (3) list of at least three of the most current past performance references for similar work covered by this solicitation. Provide the following information for each reference: (a) name and telephone number of contracting officer; (b) date of contract; and (c) brief description of the contract; (3) completed provision at 52.212-3, Offeror Representations and Certifications Commercial Items, or a completed registration on the ORCA website at https://orca.bpn.gov. Failure to provide the above listed items with your offer, may cause your offer to be considered non-responsive. Based upon market research, the Government is not using the policies contained in Far Part 12, Acquisition of Commercial items. Since this is a service provided type contract, FAR Part 37 Service Contracting, Subpart 37.6 Performance-Based Contracting shall apply and all policies and regulations. Contractor shall be in compliance with wage determinations which can be found on the internet at http://www.wdol.gov/. In accordance with FAR 52.212-1(k), the prospective awardee shall be registered and active in the SAMS database prior to award.
