- Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Requirements (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation will not be issued. The solicitation number for this requirement is 2014-Q-72519 and is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1 Simplified Acquisition Procedures. The Centers for Disease Control and Prevention intends to award a firm-fixed-price contract for the following requirement, please either fill in the dollar amounts or provide a separate excel spreadsheet containing the same information including a list of line item number(s) and items, quantities, units of measure, and options, if applicable: Background: The Toxicology Assessment Program (TAP) is a multi-disciplinary research initiative within the Health Effects Research Laboratory (HELD) and studies the causes, mechanisms, and control of adverse health effects due to workplace exposures. TAP requires a hosted data management system to collect data from multiple laboratories and host the data in a secure managed database which the TAP scientists can access in order to review and analyze. The system shall use the Internet to transmit the collected data securely back to the hosted site. Purpose: The purpose of this procurement. is to obtain a data collection and management system that allows (1) access to a hosted commercial in-life data collection and management system for use by personnel of HELD and contract laboratories carrying out HELD-directed research for one(1) year; (2) user access across multiple locations; (3) performance of statistical tests and statistical stat upload capabilities; and (4) housing of multiple HELD study types. CLIN 001: Data Collection Software • Software must have the ability to randomize data, work scheduling, bodyweights, clinical signs, generalized results. The software should streamline workflows and processes. • Integrated with other provided software programs. CLIN 002: Pathology Software • Software should create reports and comparisons of gross/histo observations, intergroup comparisons of organ weights and organ weight ratios, gross/histopathology observations, animal cross-reference, tumor summaries, combination incidence of tumors, chronological listing of tumors, cause of death summaries, major pathological findings, and can generate FDA biometrics reports. • Integrated with other software programs. CLIN 003: Histopathology Software • Software should create individual animal data reports with various fields and matrices. • Integrated with other software programs. CLIN 004: Tables and Statistics Software Creation • Software should be able to generate tables and perform statistical analysis in relation to other integrated software programs. • Integrated with other provided software programs. CLIN 005: Report Assembly Software • A document preparation program that creates templates and final reports, including data fields, automatic document updates and can create the documents in a variety of formats (i.e. PDF, MS Word, etc. ) • Integrated with other software programs. CLIN 006: Statistical Software Compatibility • Third party vendor licenses and compatibility with Oracle, SAS, and Citrix CLIN 007: Logbook. • Software that enables a form based data collection, monitoring, record keeping, and tissue tracking system. Enables surgical forms, custom forms, sampling, and histology records to be kept. This software should provide a method to improve data quality and collection which will increase productivity and accuracy. • Integrated with other software programs. Your quote shall include the following: • Total Price for the above referenced CLINs • License fees, if applicable • Training and Training Support Delivery Date: Anticipated License to begin September 1, 2014 Delivery Address: CDC/NIOSH 1095 Willowdale Road Morgantown, WV 26505-2845 Acceptance and FOB point The quote format is at the discretion of the offeror. It is the offeror's responsibility to be familiar with the applicable clauses and provisions. Clauses and provisions may be accessed via the Internet at website http://farsite.hill.af.mil. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-74. This acquisition is set aside for small businesses under North American Industry Classification System (NAICS) code 511210 and small business size standard $35.5 million. A Firm-Fixed Price purchase order will be issued in writing to the successful offeror.
