- i. "This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued." ii. The solicitation number is HHS-NIH-NHLBI-CSB-(HG)-2014-181-DLM and the solicitation is being posted as a request for proposal (RFP). iii. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-74, July 1, 2014. iv. There are no set-aside restrictions, the associated NAICS code is 334516 Analytical Laboratory Instrument Manufacturing and the small business size standard is 500 employees. The acquisition is being conducted in accordance with the procedures of FAR Part 12 and FAR Part 13. This requirement is under the Simplified Acquisition Threshold (SAT) of $150,000.00. (BRAND NAME AND EQUAL: NO REFURBISHED OR USED SYSTEM WILL BE ACCEPTED) v. Purchase Order line items: 1. Phillips CX50 Color Flow Ultrasound System: (NEW) (MFG: 2014) (BRAND NEW) CARDIAC/VASCULAR CALCULATIONS PACKAGES, PEDIATRIC CARDIAN, (TDI) Includes Tissue Doppler Imaging, Color Doppler, CW/PW Doppler, M-Mode, (CPA) Color Power Angio, Duplex and Tri-Plex, THI- Tissue Harmonics, Smart Exam, Sono CT, I Scan, Advanced X-RES, Live Compare, DICOM NETWORKING, STRESS ECHO, ECG, ECG CABLES AND LEADS, DVD/RW, Operators Manual; 2. S5-1 Adult Cardiac Transducer (NEW); 3. S8-3 Pediatric Cardiac Transducer (NEW); and 4. Shipping Charge. vi. Background : The National Institute of Health (NIH) is the nation's leading medical research agency and the primary Federal agency mission to seek fundamental knowledge about the nature and behavior of living systems and the application of that knowledge to enhance health, lengthen life, and reduce illness and disability.conducting, supporting and making medical discoveries that improve people's health and save lives. The National Human Genome Research Institute (NHGRI), Medical Genetics Branch (MGB) seeks to identify and understand inherited disorders of metabolism and of human development. MGB investigators focus on human genetics, vertebrate embryology, inborn errors of metabolism, and neurogenetic disorders. Projects performed at the biochemical, molecular, and cell biological levels involve the direct study of human subjects as well as the development and use of experimental model systems, such as zebrafish and mouse. The Branch fosters outstanding basic research and serves as a model for translational discoveries at the bedside. Branch researchers develop and test new diagnostic tests and treatments for patients with rare genetic disorders in the NIH Clinical Center. Muenke's research program seeks to improve our knowledge about birth defects and normal and abnormal early embryogenesis with special emphasis on the central nervous system, cardiovascular system, and reproductive system. Major areas of focus involve holoprosencephaly (HPE), attention deficit hyperactivity disorder (ADHD), craniosynostosis, congenital heart disease (CHD), and sex chromosome anomalies. The Muenke lab is conducting research in Nigeria, centered at Lagos University Teaching Hospital in collaboration with Dr. Ekanem Ekure. This research uses state of the art genomic technology to characterize both congenital heart disease and the acquired heart disease: rheumatic heart disease (RHD). In order to precisely diagnose and characterize heart disease in Nigeria, an echocardiography machine is needed. This instrument is expected to conduct 1200 exams/echocardiograms per year for the next three years. The CX50 ultrasound machine with adult/pediatric transducers will be used for non- invasive cardiac imaging. The CX50 system is a compact ultrasound/echocardiography system with clinically proven imaging technology. Features include all-digital compact broadband beamformer with pulse shaping capability, color Doppler imaging, and excellent image quality. The CX50 compact size allows for portability to different clinics and locations Objectives: The CX50 echocardiography unit will be used to identify and characterize heart defects in research participants for Protocol Number: 13-HG-N027 "The Genetic Basis of Congenital Heart Disease in Africa and Asia" and 14-HR-N098 "The Genetic Basis of Acquired Heart Disease in Africa." Contract Requirements : Contractor : 1. The Contractor shall be responsible for shipment of the instrument as outlined to the National Institutes of Health. Government Responsibilities : 1. The Government will keep the Equipment in the environmental conditions recommended by the manufacturer. Protect the Instrument and associated Hardware from all adverse elements, such as dirt, dust and liquids of any kind. 2. The Government will use the Equipment only in accordance with such instructions and recommendations relating to the care and operation of the Equipment as may be issued by the manufacturer of the Equipment or as may from time to time be advised in writing by the Contractor. 3. The Government will not allow any person other than the Contractor's representatives or the Customer's representative working under the Contractor's specific instruction to maintain, repair or replace any part of the Equipment. Reporting Requirements and Deliverables : The Contractor shall provide a summary report of their action after a repair and/or preventive maintenance inspection. Inspection and Acceptance Requirements: Inspection and acceptance of item will be performed after shipping Instrument to NIH. If it is a repair, Instrument needs to demonstrate continuous performance. viii. The FAR Provision 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition and is hereby incorporated by reference. All Federal Acquisition Regulations (FAR) clauses may be viewed at http://acquisition.gov/comp/far/index.html. The proposal must reference the RFP Number: HHS-NIH-NHLBI-CSB-(HG)-2014-181-DLM. All responsible offerors may submit a proposal, which if timely received, shall be considered by the agency. All proposals shall contain the following: 1. Name of Contracting Organization; 2. Solicitation number; 3. Contract Type; 4. Total Contract Value and Discounts; 5. Description of Requirement; 6. North American Industry Classification System (NAICS) Code; 7. DUNS Number; and 8. Business Size. ix. In accordance to FAR clause 52.212-2, Evaluation - Commercial Items The Government will award a Purchase Order resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Price (This will be Firm-Fixed Price): An evaluation of the offeror's price proposal will be made to determine if proposed prices are realistic for the work to be performed, reflect a clear understanding of the requirements, and are consistent with the technical proposal. x. In accordance with FAR Clause 52.212-3, Offeror Representations and Certifications -Commercial Items, offerors must complete annual representations and certifications on-line at https://www.sam.gov/portal/public/SAM/. xi. FAR clause 52.212-4, Contract Terms and Conditions - Commercial Items applies to this acquisition. xii. FAR clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items applies to this Acquisition. There are no additional contract requirement(s) or terms and conditions (such as contract financing arrangements or warranty requirements) determined by the contracting officer to be necessary for this acquisition and consistent with customary commercial practices. xiv. The Defense Priorities and Allocations System (DPAS) and assigned rating does not apply to this acquisition. xvi. All responsible offerors may submit a proposal, which if timely received, shall be considered by the agency. Offers are due by 8:00 A.M. (EST) on July 23, 2014.
