DESCRIPTION/SPECIFICATIONS/STATEMENT OF WORK 1.SCOPE: The contractor shall furnish and install all equipment and parts/supplies necessary to meet the terms and conditions of this contract in accordance with the statement of work for the procurement of an NEC 4000-lumen Widescreen Entry-Level Professional Installation Projector System. The equipment will be located at the VA North Texas Health Care System, 4500 S. Lancaster Rd., Dallas, TX 75216, Pathology & Laboratory Medicine Service Conference Room. 2.PRICING: Price is a firm-fixed price resulting from award of this solicitation. 3.PERFORMANCE SPECIFICATIONS/STATEMENT OF WORK: a.The contractor shall furnish and install all equipment and parts/supplies necessary to meet the terms and conditions of this contract in accordance with the statement of work for the procurement of an NEC 4000-lumen Widescreen Entry-Level Professional Installation Projector System. The equipment will be located at the VA North Texas Health Care System, 4500 S. Lancaster Rd., Dallas, TX 75216, Pathology & Laboratory Medicine Service Conference Room. 4.SERVICE DURING WARRANTY: a.A minimum of a one (1) year warranty after acceptance will include all parts, labor, preventative maintenance and adjustments necessary for operation of the equipment at a level meeting all requirements of the salient characteristics/specifications of this equipment. Services performed under the warranty time frame, shall be provided within the standard commercial service time frames, Monday through Friday, 8:00 a.m. to 5:00 p.m. Any "emergency" work required by the VA outside the standard commercial service time frames shall be purchased as a separate purchase order from VA North Texas Health Care System. 5.PRODUCT MODIFICATION, REMOVAL OR RECALL: a.If any product awarded under this solicitation requires modification, is removed or recalled by the manufacturer, or if any required modification, removal or recall is suggested or mandated by a regulatory or official agency, the following steps will immediately be the by the manufacturer: Notify the Contracting Officer, VA North Texas Health Care System, 4500 S. Lancaster Rd., Dallas, TX 75216, in writing, by the most expeditious manner possible. Provide two copies of the notification which shall include, but not be limited to the following: i.Complete item description and/or identification, order numbers from customers, and the contract number assigned as a result of an award on this solicitation. ii.Reasons for modifications, removal or recall iii.Necessary instructions for return for credit, replacement or corrective action. 6.SECURITY CLAUSE a.The COR and the Contractor will assure that- i.No other information except what is in this contract will be shared with the contractor in any follow up communication. ii.There will be supervised access when the contractor is physically present for technical support, service calls, and scheduled preventative maintenance. iii.NO VA DATA OF ANY TYPE SHALL BE TRANSFERRED FROM THE VA. iv.THE VENDOR WILL NOT HAVE REMOTE ACCESS TO THIS EQUIPMENT. v.All hardrives for this item will not leave the VA. vi.Non-volatile memory devices, working or non-working, shall NOT be removed from the VA facility until the ISO has certified that the data has been destroyed. For magnetic devices and media, the data destruction will be by degaussing. Other forms of cleansing will be used for non-magnetic media. vii.All electronic storage media used on non-VA leased or non-VA owned IT equipment that is used to store, process, or access VA information must be handled in adherence with VA Handbook 6500.1, Electronic Media Sanitization upon: 1.Completion or termination of the contract or 2.Disposal or return of the IT equipment by the contractor/subcontractor or any person acting on behalf of the contractor/subcontractor, whichever is earlier. Media (hard drives, optical disks, CDs, back-up tapes, etc.) used by the contractors/subcontractors that contain VA information must be returned to the VA for sanitization or destruction or the contractor/subcontractor must self-certify that the media has been disposed of per 6500.1 requirements. This must be completed within 30 days of termination of the contract. 7.RECORDS MANAGEMENT: a.All records (administrative and program specific) created during the period of the contract belong to VA North Texas Health Care System (VANTHCS) and must be returned to VANTHCS at the end of the contract.
