Description
REQUEST FOR INFORMATION (RFI) Department of Veterans Affairs Sioux Falls, SD THIS IS A REQUEST FOR INFORMATION (RFI) ONLY. This request is for planning purposes only, and does not constitute a Request for Proposal, a Solicitation, a Request for Quote or, or an indication that the Government will award a contract for the items contained in this RFI. This notice is not to be construed as a commitment on the part of the Government to award a contract, nor does the Government intend to pay for any information submitted as a result of this request. The Government does not reimburse respondents for any costs associated with submission of the information being requested or, reimburse expenses incurred for responses to this RFI. Companies are encouraged to respond if they have the capability and/or capacity to provide the items identified below. The VA Sioux Falls Health Care System (VASFHCS) has a requirement to lease an estimated 3000 square foot modular building to occupy the Integrative Health department. The VASFHCS is anticipating a five year base lease with (5) one year option periods. The lease shall include design services, site prep, delivery and installation. Site Prep shall include but is not limited to utility connections, building foundation and a connection to the existing facility. Please see attached conceptual site layout, Attachment A. The Government is requesting that interested Offerors provide responses to the following questions with their response to this RFI: 1.VA's proposed site for the modular is irregular in shape. Please address your company's capability to provide a 3000 square foot modular building that will fit in the space identified in Attachment A (Site Plan). 2.Please provide a floor plan that accommodates all required spaces identified in Attachment B (Space Requirement Plan). 3.Please provide your companies capability to provide all site prep, delivery and installation services for modular buildings. 4.Please provide any pricing information available for site prep, delivery/installation and lease costs. 5.Please provide your company's ability to provide all requested services on GSA contract. 6.Please provide an explanation of how your company will remove building once lease is completed and costs for removal. RESPONSE INFORMATION: Respondents are requested to identify their name and business size and any additional information applicable to this RFI. Submissions are due NO LATER THAN August 8th 2014 by 10:00 am Local Nebraska Time to Bridget Parizek at bridget.parizek@va.gov. Please provide any questions about the requirement to Bridget Parizek at bridget.parizek@va.gov no later than August 1, 2014. If your organization/company has the potential capability to provide the requested services please provide the following information: a.Company Name and Capability Statement, Point of Contact address and e-mail, website address, DUNS number, telephone number, business size (i.e. large, small business, service disabled veteran owned, veteran owned, woman owned, disadvantaged, HubZone, or 8(a)) as applicable for NAICS Code. If your company feels that this requirement can be fulfilled under a different NAICS Code, please provide the suggested NAICS Code and a rationale for the recommendation. DISCLAIMER: Issuance of this notice and request is to assist the VA in determining sources capable of providing a Clinical Privileges Library in accordance with the MTRs as outlined above. This notice does not commit or bind the Government under any circumstances nor does it constitute any obligation whatsoever on the part of the Government to procure the aforementioned supplies.
