- Solicitation W912P5-14-T-0083 is a Request for Quotation (RFQ) for Thirty-three (33) Sheets of Ultra High Molecular Weight Polyethylene (UHMW). This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 of the Federal Acquisition Regulation (FAR), as supplemented with additional information in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The UHMW shall be Tivar 1000 UHMW-PE. UHMW must meet ASTM D 4020-05 with a molecular weight of 3.1 to 6.2 million G/Mol, FDA 21CFR177.1520 SEC 2.1, 3A Dairy 20-23 approval, and ASTM D6712 AS S-UHMW-PEO111. UHMW resin tests must be per ISO 11542-1 test procedures. The UHMW shall be of the type normally furnished for heavy duty applications and service. UHMW shall be the product of a manufacturer that specializes in the production of this type of material as evidenced by the existence of previously published material data. All material shall be new and unused. Defective material shall be replaced or repaired as approved. The UHMW shall be factory inspected to ensure that it is free from defects, cracks, checks, etc. The UHMW shall be packed in a manner acceptable to preserve the condition of the material as it left the Contractor's facility. The packaging shall clearly identify the contents and weight of the package. All transportation, delivery, and storage costs shall be included in the quote. Material shall be adequately protected from damage during shipping and handling. All damage shall be repaired or replaced by the Contractor with no additional cost to the Government. The contract resulting from this solicitation will contain five line items, CLIN 0001 for 15 sheets of UHMW 4' x 8' x 2"; CLIN 0002 for 4 sheets of UHMW 4' x 8' x 2.25"; CLIN 0003 for 10 sheets of UHMW 4' x 8' x 2.5"; CLIN 0004 for 4 sheets of UHMW 4' x 8' x 2.75"; CLIN 0005 for all shipping and handling charges associated with CLINs 0001 through 0004 a destination of Tennessee River Operations Center, 704 South Wilson Dam Road, Florence, AL 35630-3999 (ATTN: Gerald Choat). The Contractor shall contact the Facility Manager, Mr. Gerald Choat (256-764-5223), one week prior to delivery to coordinate acceptance. Delivery will be made Monday -Thursday 0600-1630 except Federal holidays. The NAICS Code for this acquisition is 326199, All Other Plastic Product Manufacturing. The SBA small business size standard for this NAICS Code is 750 employees. This is a Small Business Set-Aside. Offerors must be registered in the System for Award Management (SAM) in order to be awarded a contract.
