- Solicitation Request for Quote The Health Resources and Services Administration (HRSA), National Hansen's Disease Programs (NHDP), is contemplating issuance of a purchase order pursuant to Federal Acquisition Regulations (FAR), Part 13. The purpose of this procurement is to purchase Patient Support Services for the National Hansen's Disease Programs (NHDP) Hansen's Disease (HD) patients located in Baton Rouge, LA. The Contractor shall perform the following tasks: 1. The contractor shall provide assistance in the Activities Department programs, including shopping and restaurant outings with patients and shopping for patients' personal needs. The contractor shall assist during holidays and special events, including assistance with patient needs. The contractor shall provide a written quarterly report of all activities within the three (3) month period of performance. 2. The contractor shall assist NHDP patients to local medical appointments, helping with routine medical office paperwork, provide bilingual (Spanish-English) assistance in communication regarding medical concerns between the patient and medical staff, and provide emotional support. The contractor shall also provide bilingual assistance with patient communication needs between patient and NHDP staff and outside service providers. The contractor shall have an exceptional command of English and Spanish and the ability to effectively communicate in both languages verbally and in writing. 3. The contractor shall be able to lift 25 pounds, bend, reach, push and pull, twist at the waist, and walk to perform the above tasks. 4. The contractor shall provide other Social Work assignments as necessary, such as keeping production statistics, organizing and developing Excel spread sheets, preparing mailings for patients and basic office reception activities. 5. The contractor shall provide the above assistance at the two Baton Rouge locations and the Carville location. The contractor shall work with elderly patients, patients with disability and patients with HD-related issues. The contractor shall provide 32.5 hours per week, Monday through Friday, except Federal holidays. The hours are from 9:00AM-3:30PM, unless it is necessary to accommodate the patient's medical appointments that are outside of these hours. The period of performance will be for a base period of twelve (12) months with one (1) twelve (12) month option period. The North American Industry Classification System (NAICS) for this requirement is 561990. The proposed simplified acquisition action is for supplies or services for which the Government intends to issue a solicitation under authority of the Federal Acquisition Regulations (FAR), Part 13. Responsible parties may submit a quote, which must be received within 15 days after the date of publication notice. This is a 100% Small Business Set-Aside.
