- AMENDMENT 0001 AMENDMENT 0001 THE SOLICITATION OFFER DATE HAS BEEN EXTENDED TO FRIDAY, JULY 25, 2014, MS. THIS AMENDMENT MUST BE ACKNOWLEDGED AND SIGNED. SEE ATTACHMENT: AMENDMENT 0001. SHOULD YOU HAVE ANY QUESTIONS, PLEASE FEEL FREE TO CONTACT ME, NADINE JOE AT 520/295-2464. This is a combined synopsis/solicitation for commercial items prepared in accordance with the FAR Subpart 12.6 and supplemented in additional information and clauses included in this notice. The solicitation number for this procurement is 14-249-SOL-00020 and is a Request for Quote (RFQ), Brand Name or Equal to the requested Digital Radiographic Equipment (FAR 52.211-6 Aug 1999). The North American Industry Classification System (NAICS) is 334517, Irradiation Apparatus Manufacturing. The Offeror must be authorized, qualified and a certified vendor and must quote on all or none basis. Written quotes are required. Oral offers will not be accepted. Only one contract will be awarded. The Contractor shall provide digital radiographic systems with all components which are Dicom Standard Reporting (S/R) allowing for transfer to our Picture Archival Communication System, (PACs) to the Tucson Area Indian Health Service, Sells Indian Hospital and the San Xavier Health Center.
