*7/22/2014 - Amendment # 2: (1) To add and notify offeror(s) that this acquisition is subject to the Service Contract Act. (2) Add Department of Labor Wage Determinations (3) Add Clause 52.222-41 - Service Contract Labor Standards. (4) Add site visit attendee(s) sign in sheet This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6-streamlined procedures for commercial items, in conjunction with FAR subpart 13 and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals/offers are being requested and a written solicitation will not be issued. Solicitation VA262-14-Q-1055 is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72. This procurement will be a 100% Set-aside for Service-Disabled Veteran-Owned Small Business (SDVOSB). NAICS Code is 238210 and the size standard is $14.0 million. Firm shall be registered and certified as a SDVOSB within www.vetbiz.gov. Any firm that does not meet the capability and size standard under this NAICS code should not submit a response to this notice. This procurement is for the installation of a 12 Strand Single Mode Backbone Fiber Optic Network for the nurse call station at the VA Long Beach Healthcare System (VALBHS). The installation of the fiber optic network will be comprised of seven (7) different buildings within (VALBHS). See the attached Statement of Work (SOW) for details and specifications. This procurement is required for the VA Long Beach Healthcare System, located at 5901 7th. St., Long Beach, CA 90822 within Veteran Integrated Service Networks (VISN) 22, in accordance with the terms, conditions, special provisions and schedules herein. The estimated period of performance shall be no more than 30 business days; and is estimated to start on or about August 1, 2014. An optional site visit has been scheduled for July 22, 2014 - 10:00 AM PST. It is highly recommended that offeror(s) attend the site visit to fully understand the requirement and clear any ambiguities pertaining to the requirement. The following information is instructions for the site visit: "Meeting Location: oVA Long Beach Medical Center 5901 7th St. Building 5 Long Beach, CA 90822 "Contractor must bring valid state issued ID for identification purposes. "All contractors must sign in on the provide sign-in sheet This acquisition is subject to the Service Contract Act of 1965. The Department of Labor of applicable Wage Determinations is provided as an attachment. Award will be made in accordance with FAR provision 52.212-2 and will be awarded based on lowest price technically acceptable (LPTA). Offeror(s) will be evaluated on a PASS/FAIL rating on the following factors: 1.Lowest Price 2.Valid business license 3.Certified and registered within www.vetbiz.gov 4.Past Performance Submitting Quote: Offeror(s) shall submit their quote on a company letterhead and shall include the completed pricing forms, along with name, address, telephone number of the offeror, firm's DUNS#, total price. Offeror(s) shall also have a valid a registration at www.sam.gov and provide a completed copy of their FAR Provision 52.212-3 - Offeror Representations and Certifications -- Commercial Items with the quote.
