Description
1. INTRODUCTION: This sources sought notice is for information and planning purposes only and shall not be construed as a solicitation or as an obligation on the part of the Department of Veterans Affairs (VA). The VA is currently conducting market research to locate qualified, experienced and interested potential sources. Responses will not be considered as proposals, nor will any award be incurred by interested parties in responding to this sources sought announcement. The VA Boston Healthcare System is seeking interested sources for a near-future solicitation and procurement. At this time, no solicitation exists. 2. BACKGROUND: The Department of Veterans Affairs' (VA) Five-Year Plan to End Homelessness Among Veterans calls for enhancing current homeless service capacity as well as developing new programs and initiatives in concert with community and federal partners. The VA has been providing direct and specialized services for homeless Veterans for over 20 years. Beginning in 1987 with 43 pilot programs that provided street outreach and residential community services, VA Homeless Programs have developed and expanded to become the largest integrated provider of homeless services in the country. Despite these resources, on any given night VA estimates that there are still over 107,000 Veterans who are homeless. 3. OBJECTIVES: The VHA in collaboration with the Department of Housing and Urban Development (HUD) are looking for SDVOSB and VOSB contractors that can provide thirteen (13) Peer Support Specialists. This requirement is part of the larger HUD-VASH Program to eliminate homelessness among our Veterans. The role of such staff will be to provide ongoing supportive services to a case load of 50 HUD- VASH Veterans each. It is preferred that all Peer Specialists are Veterans themselves in order to act as peers and build that relationship with those the program aims to help. The Peer Specialists will be at one of three VAMC locations within Massachusetts, Monday through Friday, 8am to 4:30pm. "Specific Tasks: oThe contractor shall be expected to participate as a VASH team member and engage in team activities. oThe contractor shall function as a role model to the Veterans: "Exhibiting competency in personal recovery and use of coping skills, "Serving as an advocate, "Providing consumer information and insights into healthy, recovery-oriented activities. "The contractor may utilize their own personal experiences to provide hope, encouragement, and the belief that achievable outcomes are possible for the Veterans to obtain. oThe contractor shall assist the Veterans in obtaining the needed documentation required by the PHA to include but is not limited to: "Birth certificates, "Driver's license, "Income verification, "Additional information required by housing authority and/or potential landlord. "Assist the Veterans in getting to the voucher briefing and securing a Section 8 voucher, if deemed eligible by the PHA. oThe contractor shall provide hands on/direct assistance to enable Veterans to find suitable housing. Assistance may include but is not limited to: "Transporting Veterans around neighborhoods to locate vacant apartments, "Mileage shall be reimbursed at a rate of $0.50 per mile "Helping Veterans do on-line housing searches, if locally available, "Advocating with potential landlords, property managers, and realtors, "Assisting the Veterans with obtaining their security deposit. oThe contractor shall assist Veterans in securing necessary funding for move in costs, not limited to accessing outside funding for deposits, utility deposits, and furnishings. oThe contractor shall assist Veterans in obtaining the necessary utilities to maintain the apartment, and resources such as Energy Assistance to maximize affordability. oThe contractor shall attend meetings between the Veterans and the housing authority including annual certifications, MOVE briefings, and interim appointments, if requested by the Veterans. oThe contractor shall support the Veterans' efforts to obtain any and all entitlements, pensions and resources available to assist veteran in obtaining supportive housing goals outlined in the Individual Service Plan (ISP). The ISP details the Veteran's needs and gives a set of goals they will work on in a 3, 6, or 12 month period. Referrals to Service Officers, Veterans Service Organizations, Veteran Legal Clinic and other community supports to assist in this process shall be made. oThe contractor shall assist veterans in identifying transportation resources i.e., public transportation, VA transportation if eligible, and when necessary provide transportation for clients to attend appointments related to their identified ISP goals which can include meeting with clinicians, landlords, Public Housing Authorities and various other community providers and resources. Efforts will be made to secure bus passes, subsidized transportation and resources to support Veterans reaching optimum independence. oThe contractor shall provide life skills education/support to Veterans, to allow for maintaining a safe living environment and accessing community resources including such topics as: "Tenant responsibilities in the lease and with the landlord "Monthly budgeting for rent and utilities "Apartment maintenance. "How to be a good neighbor/tenant "Relapse prevention (if applicable) "Healthy lifestyle/living, social reintegration "Identifying and accessing community resources "Vocational training and employment opportunities "Locating and participating in self-help groups "Acquiring a job o The contractor shall maintain professional boundaries with Veterans. "The contractor shall foster positive relationships between the Veterans and the VA and community programs/agencies. 4. SUBMITTAL INFORMATION: Contractors having the skills and capabilities necessary to perform the stated requirements should submit a response as detailed below. Companies should provide clear and unambiguous evidence to substantiate their capability to fulfill these requirements. Please highlight what your system would be to dispose of the waste. Interested parties shall furnish the following minimum information: 1. Company name and address 2. Company point of contact name, telephone number and email address 3. Evidence that company is a certified SDVOSB or VOSB (or in the process of certification), the number of years in business, affiliate information: parent company, joint venture partners, potential teaming partners. 4. If the company holds a Federal Supply Schedule (FSS) Contract, list the GSA Contract Number and SINs 5. Capabilities/Qualifications: A written response providing clear and unambiguous evidence to substantiate the capacity to fulfill this requirement. Description of the capabilities/qualifications/skills your company possesses to perform services described in the scope of work. Please focus on your company's recommendation for accomplishing the objectives listed above. 6. Past Experience: Brief summary of the company history relative to similar requirements. Offer may provide the following information on a maximum of three similar projects for which the responder was a prime or subcontractor. The primary NAICS Code is 624190. This is expected to be a firm-fixed price contract for one year. The set aside status is SDVOSB. All responsible sources may submit a capability statement, proposal, or quotation which shall be considered by the Contracting Officer. This is not a solicitation.
