Description

1. Scope: The Contractor shall provide for a range of specified health care services to eligible Veterans in Veterans Integrated Service Network (VISN) 05. These services will remain consistent with the VHA Patient Aligned Care Team (PACT) model which emphasizes timely, continuous and coordinated primary health care services. The contract includes the delivery of primary care services, contract management functions, as well as administrative services such as referral management, grievance handling, clinical information transfer, performance management and reporting, and clinical quality improvement activities. Veterans will access the Contractor's services when referred for care by an authorizing VA medical center (VAMC) designated as the parent facility.
