Description
This is not a solicitation announcement. This is a sources sought synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (Hubzone, 8(a), Small Business, Small Disadvantaged Business, Veteran Owned Small Business or Service Disabled Veteran Owned Small Business) relative to NAICS 811219 size standard $20.5 million. Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. Responses to this sources sought synopsis are not considered responses to any potential solicitation announcement. The Department of Veterans Affairs VHA Network 15 Contracting Office is seeking information concerning possible sources to provide preventive maintenance services per manufactures specifications for the following Magnetic Resonance Imaging (MRI) equipment. General Electric (GE) MRI Model # MSM035, GE MR 1.5T 16 Channel HDxMTI, including GE supplies coils and spectroscopy and all subcomponents to include GE MR magnet maintenance and cryogen supply, GE MR Advantage Workstation 4.4, MV PR Liebel-Flarshiem Optistar LE Injector. This equipment is a mobile MR system home based at the Dwight D. Eisenhower VA Medical Center 4101 4th Street Trafficway Leavenworth, KS 66048. The VA intends to award a contract to a qualified organization to provide these services for approximately the period of time of 09/01/2014 thru 08/31/2015 base year, with two follow-on option years. A Request for Proposals will be posted on FebBizOpps after the VA concludes its market research to the availability of possible sources. These services have been provided by expiring contract VA255-P-1158. All interested firms shall provide the following information: company name, address, point of contact, phone number, email address, capability statement, business size and socio-economic classification. Telephone responses will not be accepted. Responses must be received in writing no later than 4:00 p.m. CST July 28, 2014. This notice is to assist the VA in determining sources only. A solicitation is not currently available. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. Responses to this sources sought is not a request to be added to a prospective bidders list or to receive a copy of the solicitation. See Statement of Work and Task Description below: Section C - Description / Specifications 1.The contractor shall provide full maintenance on site, to include all replacement parts, cryogens, software updates, labor, travel expenses, tools, test equipment and other incidentals necessary to maintain or restore the listed equipment to operating specifications which meet or exceed the manufacturers' specifications. Any and all parts used must be new or factory rebuilt for that model and type of device. If any action requires replacement of hard drive (or other storage media), the contractor shall not remove the hard drive (storage media) from the VA premises and, must turn over said device to Biomedical Engineering. In-house Biomedical Engineering will provide first look response to all trouble calls and will notify contractor when services are needed. The contractor must respond by telephone for diagnostic response within two (2) hours after notification, and must respond on the premises of the VA Eastern Kansas Health Care Systems (EKHCS) campuses (in this case the Leavenworth VA Medical Center) within one (1) business day when required for on site service call as requested by the Contracting Officer Representative (COR) or a designee. 2.All services for this contract must meet manufacturers' performance and technical specifications, ISO standards, Federal Regulations, FDA device certification and other specifications that may apply such as National Electric Code (NEC), Life Safety Code (LSC), the Joint Commission (JC), and VA Regulations. Also, Contractor must provide the appropriate service documentation at no additional expense. Contractor shall provide hardcopy proof, upon request by COR, that contracting entity is a certified manufacturer service provider and that technicians are current in device certification training. Contractor service representative shall report to the Engineering Offices upon arrival on station to be issued a temporary VA Contractors ID badge and shall wear the VA issued ID badge above the waistline at all times while on the premises. 3.The contractor must provide hardcopy of the national BAA and Site to Site VPN agreement to the COR." The devices do have VPN access. This system does link into the VISTA computer network and PACS thus the contractor shall have access to the VISTA computer network and could have access any PHI and or PII (electronic or hardcopy) at anytime while servicing the system. 4.The contractor shall ONLY respond to service calls placed from COR or designee. The Contracting Officer Representative (COR) for this contract is Carl Campbell, his office phone number is (913) 946-1586 and his email address is Carl.Campbell@va.gov. Contractor shall not perform any service that will result in additional charges without prior approval from the VA Hospital Contracting Officer. Contractor shall include all hardware and software, updates, patches, and revisions, and licenses to already purchased devices and options at no additional charge to the VA EKHCS, Leavenworth campus. 5.All contractor personnel must report to the COR or, the Biomedical Engineering Section to sign in before any work is performed on the premises of the VA EKHCS, Leavenworth campus, and to sign out after completion of assignment. The contractor must furnish a detailed field service report, or equivalent, showing work completed, listing all repairs and/or repair parts used to maintain operating efficiency of the equipment. This report must be delivered to Biomedical Engineering as the contractor signs out and leaves the facility or via email to Carl.Campbell@va.gov. If service is completed after 4:30 pm, sign out and fax the report to the Biomedical Engineering Department, (FAX # (913) 758-4230). Payment will be withheld pending receipt of this report. The service report from the contractor must contain, at a minimum, the following information: a.Date and time of the contractor's arrival on station. b.Type, model, and serial number(s) of all equipment on which maintenance was performed. c.Total time spent performing maintenance, excluding travel time. d.Detailed narrative description of the malfunction or PM action required. e.Complete list of parts replaced. f.Comments as to the cause of the malfunction when applicable. g.Date and time the repair or software upgrade was completed. 6.There is a potential for exposure to blood borne or other infectious material with equipment throughout the hospital. All maintenance persons must use the "Universal Precautions" (i.e. decontamination of medical equipment, wearing protective gloves, aprons, and goggles) during cleaning and maintenance/repair procedures required by this contract. 7.Payment will be made quarterly, in arrears, upon receipt of a properly prepared invoice, referencing the period for which payment is due, contract number and/or purchase order number assigned. Preventive maintenance inspections shall be performed in accordance with manufacturer's literature on a quarterly basis and must be prescheduled to avoid equipment access conflicts. Preventive maintenance inspections shall be performed in a manner which clearly demonstrates that all equipment meet or exceed all manufacturers' technical performance specifications for preventive maintenance. Each preventive maintenance inspection shall include all items outlined in the manufacturers' manual. 8.Equipment Covered by this contract includes: "Nomenclature: Scanning Systems: MRI "Manufacturer: GE Healthcare "VA ID# 589 EE94115 "Serial# 1S9FA482081183286 "Model# MSM035, GE MR 1.5T 16 Channel HDxMTI, including GE supplied Coils and Spectroscopy "And all sub-components i.GE MR Magnet maintenance and Cryogen supply. ii.GE MR Advantage Workstation 4.4. iii.MV PR Liebel-Flarshiem Optistar LE Injector. "This equipment is a mobile MR system home based at the Leavenworth VA Campus. 9.At the completion of each repair, the Contractor shall provide a written service report to the COR or his designee. This report shall clearly state the date of service, type of service performed, the model and serial number of equipment serviced, and the name of the field technician who performed the repair. Service reports will be used to verify work. Payments will not be made without service reports to document work.
