- Wage Determination Peformance Work Statement Request For Quote In accordance with FAR 12.603(c)(2): This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotation, which incorporates provisions and clauses for commercial items in effect through Federal Acquisition Circular 2005-74. The Government intends to award a Firm-Fixed Price Contract based on Lowest Price Technically Acceptable. To be determined technically acceptable, offerors must provide evidence of recent and relevant past performance. To be considered recent, past performance must have occurred within the past 5 years. To be considered relevant, past performance must have been for work in similar scope and magnitude. T he Government will evaluate offers for award purposes by adding the total price for all options to the total price for the base year. Evaluation of options will not obligate the Government to exercise the options. The associated NAICS code for this acquisition is 238210 with a size standard of $14.0M. McConnell AFB, KS 67221-3702 plans to acquire the following project: ARW Camera Maintenance Item Description QTY Unit 0001 Base Year Inspection, Cleaning and Repair of Security Cameras in accordance with Performance Work Statement, dated 2 May 2014. Period of Performance: 15 September 2014 - 14 September 2015. 12 MO 0002 Base Year Parts Draw in accordance with section 1.2 of the Performance Work Statement, dated 2 May 2014. TBD LT 1001 Option One Inspection, Cleaning and Repair of Security Cameras in accordance with Performance Work Statement, dated 2 May 2014. Period of Performance: 15 September 2015 - 14 September 2016. 12 MO 1002 Option One Parts Draw in accordance with section 1.2 of the Performance Work Statement, dated 2 May 2014. TBD LT 2001 Option Two Inspection, Cleaning and Repair of Security Cameras in accordance with Performance Work Statement, dated 2 May 2014. TBD LT *TBD - To be determined by the Government In accordance with FAR 52.219-6, Notice of Total Small Business Set-Aside, this is a notice that this order is a set-aside for small business concerns. Only quotes submitted by small business concerns will be accepted by the Government. Any quote that is submitted by a contractor that is not a small business concern will not be considered for award. Award will be made to the responsive offeror determined to be the most advantageous to the government in accordance with the criteria listed in the attached RFQ. Awardee will be required to invoice through Wide Area Work Flow (WAWF). Award can only be made to contractors registered in System for Award Management ( http://www.SAM.gov ). In accordance with AFFARS 5352.201-9101, Ombudsman, (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions). (c) If resolution cannot be made by the contracting officer, the interested party may contact the ombudsman, [Insert names, addresses, telephone numbers, facsimile numbers, and e-mail addresses of Center/MAJCOM/DRU/AFISRA ombudsman/ombudsmen]. Concerns, issues, disagreements, and recommendations that cannot be resolved at the Center/MAJCOM/DRU/AFISRA ombudsman level, may be brought by the interested party for further consideration to the Air Force ombudsman, Associate Deputy Assistant Secretary (ADAS) (Contracting), SAF/AQC, 1060 Air Force Pentagon, Washington DC 20330-1060, phone number (571) 256-2395, facsimile number (571) 256-2431. (d) The ombudsman has no authority to render a decision that binds the agency. (e) Do not contact the ombudsman to request copies of the solicitation, verify offer due date, or clarify technical requirements.
