ATTACHMENT A STANDARD FORM 18 (SF18) STATEMENT OF WORK SOLICITATION TO OBTAIN AN INDEPENDENT SCIENTIFIC ASSESSMENT OF THE HIGHWAY 101 CULVERT AT RINCON CREEK FOR THE PURPOSE OF ESTIMATING THE EFFECTS OF THE CULVERT ON UPSTREAM MIGRATION OF ENDANGERED STEELHEAD. (I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (II) This solicitation is issued as a request for quotation (RFQ). Submit written quotes on RFQ Number NFFP5000-14-01738JR. (III) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-75. (IV) This solicitation is being issued as UNRESTRICTED. The associated NAICS code is 541620. The business size standard is $15.0 million. (V) This solicitation is to obtain an independent scientific assessment of the highway 101 culvert at Rincon Creek for the purpose of estimating the effects of the culvert on upstream migration of endangered steelhead: Line Item 0001 - Services, non-personal, to provide all labor, material and equipment (unless otherwise provided herein) necessary to obtain an independent scientific assessment of the highway 101 culvert at Rincon Creek for the purpose of estimating the effects of the culvert on upstream migration of endangered steelhead: accordance with the Statement of Work. (VI) Description of requirements is as follows: See attached Statement of Work which applies to this procurement. (VII) THE PERIOD OF PERFORMANCE SHALL BE AS FOLLOWS: August 5, 2014 through February 4, 2015 (VIII) FAR 52.212-1, Instructions to Offerors -- Commercial Items (APRIL 2014), applies to this acquisition. The offeror shall submit a quote for each line item in Section (V) above on the attached Standard Form 18. NOTICE TO OFFERORS: Instructions for submitting quotations under this request for quote must be followed. Failure to provide all information to aid in the evaluation may be considered non-responsive. Offers that are non-responsive may be excluded from further evaluation and rejected without further notification to the offeror. Submit quotations to the office specified in this solicitation at or before the time specified in the solicitation. Email quotes are preferred and can be sent to Joy.A.Richardson@noaa.gov. At a minimum the contractor shall provide the following information: a) Point of contact name, telephone and E-mail address. b) DUNS Number c) At least two references of similar projects including company name, point-of-contact, phone number and email address (if available) Provide all evaluation criteria in accordance with 52.212-2 in this package. 1352.215-72 Inquiries (Apr 2010) Offerors must submit all questions concerning this solicitation in writing to Joy.A.Richardson@noaa.gov. Questions should be received no later than noon. MDT, July 30, 2014. Any responses to questions will be made in writing, without identification of the questioner, and will be included in an amendment to the solicitation. Even if provided in other form, only the question responses included in the amendment to the solicitation will govern performance of the contract. (End of clause) (IX) FAR 52.212-2, Evaluation - Commercial Items (Jan 1999), applies to this acquisition. Offers will be evaluated based on Best Value and the factors set forth in paragraph (a), and award will be made to the firm offering the Best Value acceptable to the Government. Paragraph (a) is hereby completed as follows and the evaluation will be based on the following. Offers will be evaluated solely on Best Value technically acceptable, on an all or none basis. 1) Past Performance showing capability to meet the requirements of the Statement of Work. Two references are required with company name, point-of-contact name, phone number and email address. References must be of similar projects to be accepted. Offers received without references will be considered non-responsive. 2) Price. (X) The offeror shall complete the annual representations and certifications electronically in System for Award Management at https://www.sam.gov/portal/public/SAM/.
