Description
Evaluation Criteria for Quality Rating System Beta Test Measure Set SOW for Quality Rating System Beta Test Measure Set This is a Request for Quotation prepared in accordance with FAR Part 13, as supplemented with additional information included with this notice. This Request for Quote (RFQ) is being set aside for small business concerns. A quote is being requested and a written solicitation will not be issued. Requirement number OA-393-2014-0350 is issued as a Request for Quotation. The Centers for Medicare and Medicaid Services (CMS), Office of the Center for Clinical Standards and Quality (OCSQ) has a requirement for a contractor to provide Pharmacy Quality Alliance (PQA) measure specification, measure maintenance and technical support for the following measures that are required as part of the 2015 Qualified Health Plan (QHP) Quality Rating System (QRS) beta test. A detail Statement of Work and Deliverables are attached. This will be a fixed price procurement using simplified acquisition procedures (limitation of $150,000), at a firm fixed labor rate. The period of performance will be a 12 month period from approximately September 1, 2014 thru August 31, 2015. Responses must be sent electronically in MS Word format to phillip.harrell@cms.hhs.gov no later than 3:00pm on August 11, 2014. Responses shall be no more than 15 pages and may include the following: 1) Contractors brief capabilities overview; 2) Past performance information (relevant experience to perform the required services); and 3) Price of the deliverables. All responses will be evaluated on a best value basis using the attached criteria: 1) Technical and Management Approach - the likelihood of effectively meeting requirements; 2) Qualifications of Personnel; 3) Organizational Experience and Expertise and 3) Price - the amount, realism and consistency of the evaluated price. The Government intends to award an order to the responsible offeror whose quotation offers the best acceptable combination of technical capabilities and proposed price; the best value (considering price and other factors). All responses submitted shall remain valid for a period of 180 days. Interested parties must be able to affirm no conflicts of interest with parties involved in Medicare claims processing in light of the work identified in this Statement of Work. In the event that the vendor/government identifies a conflict or the appearance of a conflict, the vendor shall be responsible for providing a mitigation strategy that satisfies the government. In the event that the conflict cannot be resolved, the order cannot be awarded or an existing order may need to be terminated. All contractors are hereby advised that contractors shall be registered in the System for Award Management (SAM) which was formally the Central Contractor Registry (CCR). Your quote should include the following additional information: 1. Company Name and Current Mailing Address. This should be the same as the information appearing in (SAM). 2. Contact Person, name, telephone number, e-mail address 3. Business Size Classification, i.e. Small Disadvantaged, 8(a), Woman-Owned, etc. 4. DUNS Number 5. Federal Tax ID No. Questions regarding this requirement must be submitted in writing no later than 3:00pm on August 7, 2014. All quotes must be received by August 11, 2014 no later than 3:00pm.
