Description
NOTICE OF INTENT TO ISSUE SOLE SOURCE AWARD The Department of Health and Human Services, Program Support Center (PSC), Acquisition Management Services (AMS), on behalf of the Indian Health Service (IHS), Phoenix Area intends to negotiate and award a sole-source contract under the authority of FAR Part 6.302-1, 10 U.S.C. 2304(c) (1), to The Joslin Diabetes Center (JDC), 1 Joslin Place, Boston, MA 02215. This procurement will be awarded under Part 15 of the FAR Contracting by Negotiation. Supplies and/or Services Required: Background The Joslin Vision Network (JVN) is a specialized teleophthalmology modality for the remote diagnosis and management of diabetic retinopathy. JVN is a proprietary telemedicine software and clinical system developed by the Joslin Diabetes Center (JDC). It operates on the Comprehensive Diabetes Management Program (CDMP) platform, also a proprietary software system developed by JDC. The CDMP is a suite of software applications that provides multiple functions critical to the clinical operation of the JVN including patient clinical record management, database maintenance, image review/diagnosis, clinical reporting, quality assurance (QA), IHS Resource and Patient Management System (RPMS)/ Electronic Health Record (EHR) interoperability http://www.ihs.gov/rpms/ http://www.ihs.gov/ehr/index.cfm?module=technicaloverview, and DICOM based image management. Operation of the JVN system in the Indian Health Service (IHS) is done as the IHS-JVN Teleophthalmology Program, with fixed and portable implementations at 96 sites in 25 states. Image acquisition for the IHS-JVN Program is achieved with the Topcon NW6S and the Optos Daytona. The Topcon retinal imager has been modified by JDC to operate with low illumination and is equipped with an E-i-RIC MegaVision camera back producing DNG images for native processing by the JVN software. Project Purpose The JVN/CDMP technology is used to acquire, transmit, store/retrieve, archive, interpret, and report retinal images from American Indians and Alaska Natives with diabetes. Image acquisition is performed using JVN IA4 or Global Retinal Imager (GRI) software developed by JDC. Evaluation of these images by readers at the IHS-JVN National Reading Center is done using the JVN reading and reporting software providing computer assisted diagnosis of level of DR and diabetic macular edema (DME), and development of a treatment plan consistent with established standards of care. As a contingency of its use in the IHS, the system is validated and operated at American Telemedicine Association (ATA) Category 3. This project requires onsite and remote deployment services, training, imager and reader staff support, reporting, and QA for all its telemedicine operations to achieve this clinical outcome. Description of Work: The contractor provides or specifies all hardware, software, accessories, and other components necessary for the deployment and efficient operation of the JVN technology. The contractor also supplies development and maintenance of JVN/CDMP, all RPMS/EHR integration operational support, IHS specific development of the JVN imaging and reading software and hardware in collaboration with the IHS, and Topcon and Optos imager repairs and preventative maintenance. This includes: • Create and maintain a standards based electronic medical record associated with each image that interfaces with the IHS RPMS/EHR system using the CDMP hosted interface engine. • Specify all critical requirements, test, and certify imaging and reading workstations for compatibility with JVN IA4, GRI and JVN/CDMP. • Transmit stereoscopic color digital retinal images to an IHS staffed reading center using store-and-forward mode through the CDMP software platform using DICOM. • Provide and maintain software solutions for standardized multi-mode (stereoscopic, red-free, thumbnail, isolated, standards comparison, image rendering, etc.) grading/reading of the images yielding a computer assisted rules based diagnosis and treatment/follow-up plan based upon the Early Treatment Diabetic Retinopathy Study (ETDRS) and other evidence based models. • Integrate the JVN/CDMP into RPMS/EHR with bidirectional standards based interoperability • Collaborate with OIT software development teams to optimize bi-directional RPMS/EHR-CDMP interoperability including additional telehealth service delivery modules such as Remote/Home Monitoring, and creation of interfaces between RPMS/EHR/VistA Imaging (VI) and JVN/CDMP. • Maintain and upgrade the diagnostic display stations, including both fixed and mobile platforms onsite at existing sites as needed • Develop, test, and provide ongoing enhancements of an interface between the Optos Daytona SLO fundus camera and JVN Global Retinal Imager (GRI) • Upgrade existing Image Acquisition Stations to JVN GRI (WIN7 OS) using the Topcon/Megavision or Optos Daytona technology • Maintain and demonstrate CDMP IHE-Eye Care conformance with relevant profiles through the IHE Eye Care Connectathon including all software development, testing, and demonstration leading to a published IHE Eye Care conformance statement. This includes expansion of the scope of IHE Eye Care conformance as appropriate and needed by IHS-JVN customers. • Development and deployment of a CDMP/JVN integrated Reader QA system that allows seamless expansion in the future to include end-to-end QA at the National Reading Center. • Upgrade of existing Imaging Workstations to include the latest version of Mobile Reader system (HIPAA conformant and limited to read only) to allow IHS-JVN sites to view their retinal images (and only their retinal images) in a non-diagnostic presentation state. • Onsite service and support including introduction, orientation, staff training (clinical, technical, business), and troubleshooting of operational problems following deployment. • Ongoing program support and maintenance including training, mult-tier help desk, troubleshooting, JVN Imaging and Reader deployment, and software and hardware development. • Repairs to imaging devices, camera backs, and related imaging components under an annual maintenance agreement. • Maintain a loaner equipment program (Topcon NW6S with MegaVision camera back and TL 301 relay lens). • Refurbish existing Topcon NW6S cameras and TL-301 relay lenses to new service limits and warranty as needed. • Purchase, delivery, and setup of Optos Daytona SLO retinal camera for targeted replacement and redeployment of Topcon NW6S units • Training of IHS-JVN staff to provide entry level field based support of the Optos Daytona SLO retinal camera. • Identify, develop, test, and validate advanced retinal camera systems (if available) for future IHS-JVN tele-medical deployments. • Develop and maintain an externally pointing Web site for the IHS-JVN Teleophthalmology program in collaboration with the IHS web development division • Develop and maintain an internally pointing Web site on the CDMP server for the IHS-JVN Teleophthalmology program to support end-users of the program. The proposed contract is for services and equipment for which the Government intends to solicit and negotiate with only once source under the authority of FAR Part 6.302-1(A)(2). 10 U.S.C. 2304(c)(1) The purpose of this synopsis is to determine whether there may be other sources with the necessary expertise, qualifications, and experience to perform the work described above. Interested parties who feel that they are capable of fulfilling this requirement may identify their interest by providing clear and comprehensive information, related to the minimum requirements listed in the synopsis description of work, supporting their claim. The applicable North American Industry Classification System (NAICS) code is 541512, Computer Systems Design Services - $25.5 size standard. This notice of intent is NOT a request for competitive proposals. No solicitation document is available; therefore, requests for such documents will be considered invalid responses. However, all information received within 10 days of the publication of this notice will be considered by the Government. A determination by the Government not to compete this requirement is solely within its discretion. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. If no responses are received within 10 days, the Government shall negotiate and award a contract to the Joslin Diabetes Center.
