- The Bureau of Indian Affairs, on behalf of the Bureau of Indian Education (BIE), intends to issuing an RFQ to award a Firm Fixed-Price contract for Speech Therapy Services for Native American students at the John F. Kennedy Day School, located in Cedar Creek Community, 18 miles south of Whiteriver, Arizona, from August 11, 2014 through June 30, 2015. The school is located in a semi-remote area on the Fort Apache Indian Reservation. It is estimated at 72 days of services for the school year. The NAICS Code for this professional service is 621340. The contract is a Set-Aside Small Business. This Solicitation/RFQ# A14PS00831 (dated 7/30/14) will be the only notice issued; closing date is 08/08/14. All interested parties with a valid DUNS# must be registered in System for Award Management (SAM), a GSA website, for consideration for this award. QUOTE: 72 Days @ $_______ = $________ Services to students will include direct speech therapy by the Speech/Language Pathologist to meet the requirements of Federal IDEA and school policies as they pertain to individual student needs, evaluations, reports, documentation and progress reports. The Speech Pathologist must have a Master's Degree in Speech Therapy, a Valid Arizona Speech Therapy Licensure, and a Valid Arizona Driver's License; must maintain National Certification through the American Speech and Hearing Association as well as a medical license through the AZ Department of Health. Contractor will provide a direct services log for each student and each site visit and will be asked to meet with the Special Education Coordinator and Business Office at the beginning of the contract. All student files will remain on campus. Contractor will sign in with the Special Education Coordinator at each visit. The therapist must maintain a full record of students and the services received; observes, screens, evaluates students and provides documentation of student referrals; attends and participates in multidisciplinary team meetings; develops and/or implements therapy plans including a narrative description of student's exact needs and specific, measurable individual goal statements; mains service/documentation log of exact services provided to each student; provides supervision/direction to teaching assistant who provides direct services to students once per week; identifies equipment needs and makes recommendations of needs to the Special Educ Coord; completes student reevaluations and initial evaluations in accordance to BIE timelines; provides written evaluations reports to the multidisciplinary team prior to the team meeting; and provides training and support as needed to staff and parents. All prospective contractors must submit a resume and statement of qualifications, along with certificates in their field of expertise.
