- EURRC Provisions and Clauses (P&C) 14Q0363 Provisions (Required) 14Q0363 CLIN Structure (Required) 14Q0363 Request for Quote (RFQ) * See attachments for the complete detail General Overview: The Uniformed Services University of the Health Sciences (USU), located in Bethesda, MD, has a requirement for eight laboratory glassware washers. The USU is the only Federal health sciences university and we are committed to excellence in military medicine and public health during peace and war. We provide the Nation with health professionals dedicated to career service in the Department of Defense (DoD) and the United States Public Health Service (PHS) and with scientists who serve the common good. We serve the uniformed services and the Nation as an outstanding academic health sciences center with a worldwide perspective for education, research, service, and consultation; we are unique in relating these activities to military medicine, disaster medicine, and military medical readiness. USU is located in Bethesda, MD on the grounds of the Naval Support Activity Bethesda. Additional information regarding USU and its mission can be found on our website: www.usuhs.mil. This is a combined synopsis/solicitation for commercial items in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a Request for Quotes (RFQ) for commercial items in accordance with the procedures of FAR Part 12 Acquisition of Commercial Items, Part 13 Simplified Acquisition Procedures, and FAR 13.500(a). This RFQ incorporates all provisions and clauses in effect through Federal Acquisition Circular FAC 2005-74, effective July 1, 2014 and Defense Federal Acquisition Regulation Supplement (DFARS), effective June 24, 2014. This requirement is set-aside for small business concerns under North American Industry Classification System (NAICS) Code 334516 Analytical Laboratory Instrument Manufacturing with a size standard of 500 employees. This NAICS Code meets the Non-Manufacturer Rule Waiver. Small business resellers may offer other-than-small manufacturer's products. In order to be eligible for award, offerors must be registered on SAM.gov with completed Representations & Certifications (Reps & Certs). Offerors not registered on SAM.gov with completed Reps & Certs will not be eligible for award. Offerors interested in responding to this requirement must provide a quote for eight laboratory glassware washers, which at a minimum meet the specifications listed below. A single firm fixed-priced (FFP) purchase order will be awarded. Minimum Specifications: USU intends to replace eight laboratory glassware washers. We prefer the Steris Reliance 400; however, offerors are allowed to present comparable, replacement brands and models as long as they meet the following minimum specs: 1.Must be able to remove the existing 8 glassware washers and install the new 8 glassware washers; 2.Research-grade glassware washing (offerors must state the temperature, mechanical action, and chemical action specifications of their offered models); 3.Versatility to clean various types of glassware, e.g. glass pipettes, narrow-neck beakers, wide-mouth beakers, etc.; 4.Chamber size: At least 21"W X 25"H X 24"L; 5.Manual tempered glass door(s) to allow visibility into the chamber; 6.Single vertical or drop-down door cabinet; 7.Direct, forced-air, HEPA-filtered drying capability; 8.Preprogrammed descaling and priming cycles for routine maintenance; 9.Safety door switch and other safety features, e.g. door cable safety latch; 10.Integral steam or all electric operation, the heating option switch must be software-controlled; 11.High wash and de-ionized rinse temperatures (up to 95°C) for effective cleaning; 12.Multi-circulation pump system to ensure wash coverage; 13.Electrical: 120/208V, 60Hz, 3-phase; 14.At least one year of full-coverage warranty (the period of performance (PoP) for the warranty will begin the day the units are installed in the labs); 15.English-language controls; and 16.Must be able to provide targeted maintenance training to USU on-site technicians. If a vendor cannot provide this service directly, the vendor must at least be able to organize and refer us to the manufacturer/other authorized third-party vendors that can provide this training. Interested offerors should present the cost of optional add-ons and products, e.g. extended warranty, detergent, multi-level shelves, load cart, etc. Quote must clearly state what lines are the options. USU requires the awardee to demo and remove the existing washers and install the new washers. USU may, however, decide to keep the existing washers; if so, the awardee must demo and remove the existing washers and move them to a designated area on the University campus. We will decide on this before the award is made. If the price of removal is expect to differ depending on whether we decide to keep the existing units or not, interested offerors may present multiple pricings for both scenarios (1. USU requires the awardee to demo and move the existing units to a designated area for our possession or 2. Awardee must completely dispose of the units off-site). Offerors responding to this requirement must provide pricing, at a minimum, for the following contract line item numbers (CLINs): CLINDescriptionQuantityUnit of Issue 0001Laboratory glassware washers8Each 0002Removal of existing washers8Each 0003Installation of new washers8Each In order to be considered for award, offerors must provide a quote for the CLINs listed in the CLIN Structure attachment. A fully completed CLIN Structure spreadsheet, including unit price, total price, and country of manufacture must be incorporated in the quote package for consideration. Site Visit: One scheduled site visit will be held on August 5, 2014 at 2:00PM (Jerry, you can choose the date and time) at the USU, Building A, Room 1040C. All interested offerors are encouraged to attend. The site visit is subject to FAR subpart 52.237-1. Attendees should allow adequate time to gain access onto the Government facility. Due to heightened security measures, everyone entering the base must have a Federal-Government-issued ID card or a military ID. Otherwise, the offeror must be placed on the access list; those who request to be placed on the access list must provide (1) name of the attendees, (2) company name, and (3) a phone number by 12:00PM (noon) on August 1, 2014. Evaluation / Award Detail: The Government intends to award a single firm-fixed-priced (FFP) purchase order to the offeror representing the best value to the Government based on the evaluation factors listed below. Evaluation factors are listed in order of importance, with most important listed first. The Government will conduct a trade-off between price and non-price factors. The Government is not required to award to the lowest priced offeror or the offeror quoting the highest technically rated item. Evaluation factors for the electrometer are as follows (in order of importance): 1.Technical. Offerors must provide product literature, which demonstrates the capabilities of the quoted item(s). At a minimum the product literature must demonstrate that the quoted item meets the specifications outlined above. Information on warranty, training, installation, and delivery lead-time must be included in the quote submission and will be evaluated. The Government reserves the right to utilize technical (e.g. trade magazines) and customer reviews/references as part of the technical evaluation. 2.Corporate Experience. Offerors are required to provide a narrative of their corporate experience in removing and installing the laboratory glassware washers. 3.Price. Offerors must include all applicable costs (e.g. components, removal, installation, and shipping) in their quote. The total quoted price for all components will be the evaluated price. 4.Past Performance. The Government may evaluate the offeror's present and past performance on similar tasks within the past three years. Offerors must provide two (2) customer references for similar work performed. The Government reserves the right to select which reference(s) to contact. The following information must be provided for each reference: a.Name of client and address; b.Name of current PoC, including title, phone number, and email address; c.Contract number, if available; d.Contract value (total cost); e.Contract period of performance (PoP); and f.A clear description of the service provided. Vendors must indicate whether they accept credit card payments and whether this would incur additional fees. USU cannot make a payment via third-party-payment merchants, e.g. PayPal. Quoted items must be new. Quotes for refurbished equipment will not be considered. Offerors must indicate in quote where item is manufactured. Offerors may provide multiple quotes for different models that meet the required specs. Items will be shipped to Bethesda, MD. See attached list for applicable provisions and clauses for this requirement. In the preparation of the quote, please be advised that USU requires the quote to be valid until September 30, 2014. The following clause applies to this contract: 52.232-18 Availability of Funds The Government reserves the right to award without discussions. Questions/clarifications regarding this solicitation must be submitted via email to andrew.lee@usuhs.edu by 14:00 EST on August 6, 2014. Any question received after 14:00 EST on August 6, 2014 will not be answered. Questions will not be answered over the phone or submitted by any other means. All questions will be answered in an amendment to the solicitation posted on or around August 8, 2014. Quote packages are due by 22:00 EST on August 15, 2014. Quotes must be submitted via email to andrew.lee@usuhs.edu. Quotes will not be accepted by any other means. Late quotes will not be considered. Quote packages should include a cover sheet that provides the following information: a.Official Company Name; b.Point-of-Contact, including name, email address, and phone number; c.DUNS Number; d.If applicable, a statement that includes acknowledgement of latest amendment number; and e.Tax Identification Number (TIN). TIN may be used by the Government to collect and report on any delinquent amounts arising out of the contractor's relationship with the Government (31 U.S.C 7701(c)(3) - FAR 13.106-3(e)).
