*****Begin Text Document- 'FBO-DESCRIPTION' ***** This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement is being issued as a request for proposal (RFP) and constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular #2005-71 Effective 25Nov2013. This combined synopsis/solicitation is being competed on a 100% Small Business (SB) Set Aside. This acquisition will be competed to the full extent required by regulation. The NAICS code that applies is 812332 and business size is $35.5 Million. This requirement is for: Contractor shall provide all labor, materials, equipment, transportation and supervision necessary to provide inventory of mats in styles, sizes and quantities; and ongoing cleaning, maintenance and replacement services to the Dorn VA Medical Center IAW the statement of work. Quote submitted must at a minimum Meet or Exceed Specification for services requested. Each quote will be evaluated by Lowest Price Technically Acceptable (PLTA), Quality of Matting Offered, References (Similar large facilities) and Management Plan (how and who will coordinate contract services) Note: Please review the solicitation document in its entirety and the Statement of work for detail instruction on what is required prior to submitting your proposal and how it will be evaluated. Failure to provide require documentations (in accordance with SOW and Evaluation Factors), your proposal will be considered none responsive and will not be considered for evaluation or award. This is a Lowest Price Technically Acceptable (LPTA) acquisition. The Government will make an award to the responsible offeror whose offer conforming to the solicitation is the most advantageous to the government.
