- Draft PWS 1) Type of Notification: Sources Sought (FBO) This is a sources sought synopsis only, (not a pre-solicitation notice pursuant to FAR Part 5). The purpose of this notice is to gain knowledge of potential qualified sources. Responses to this notice shall include the following: (a) company name (b) address, (c) point of contact, (d) "number of employees" as defined by the Federal Acquisition Regulation (FAR) part 19.101 and (e) the amount of "annual receipts" as defined by the FAR part 19 (f) Business size. In addition, the respondent must provide a capability statement that addresses the organization's qualifications and ability to perform as a prime contractor the work described below. 2) Description: Personal Services (Dental Researcher/ Comprehensive General Dentist) The following is a brief summary of the primary duties that the Contractor must perform: The Contractor shall provide personal services for Dental Research/Comprehensive General Dentistry referred to as the Health Care Providers (HCPs) at San Antonio Texas Department of Defense Military Treatment Facilities (MTF). The HCPs shall perform services and support compatible with the medical facility's operating capacity and equipment. 3) Hours of Performance: Routine services shall be performed in the MTF during normal duty hours from 0700 and 1600 Monday through Friday. The standard duty week will be (40) hours in a five (5) day calendar cycle Monday thru Friday with 1 hour off per day for unpaid lunch. All hours in excess of 40 hours per week, are deemed emergency and require approval by the Dental Clinic Commander. 4) Qualifications: 4.1) The CP shall meet the following education requirements: Possess a Medical Degree (M.D.) from an accredited University. Current board certification by the American Board of Ophthalmology as well as documented formal fellowship training in Pediatric Ophthalmology. The Dental Researcher/Comprehensive General Dentist must provide certificate of completion from any Advanced Education in General Dentistry residency recognized by the ADA Commission on Dental Accreditation, Board Certification by the American Board of General Dentistry, and advanced fellowship training in Dental Materials. The HCPs shall keep their license/registration/certification current. Required continuing education for CDE shall be at no additional cost to the Government. The HCP shall provide appropriate CME documentation to the COR or designated Government representatives within one month after all CDE accredited events. 4.2) License and Certification Verification. The HCP must possess and maintain an active current; valid; and unrestricted license from a U.S. jurisdiction before practicing independently within the defined scope of practice for their specialty. The HCP shall have and maintain a current Basic Life Support (BLS) certification. The CP shall perform all primary and annual licensure or certification document verifications. The CP shall maintain these certifications in a current status throughout the life of this contract. The CP shall confirm that the clinical licenses or certifications have not been suspended or revoked by any healthcare facility, public or private, anywhere in the world. This does not preclude any CP whose licensure or certification, although originally suspended or revoked by a healthcare facility, were subsequently fully reinstated by the same. Healthcare professionals who fail to maintain a license, registration or certification will be removed from independent practice and privileged providers will be placed in abeyance. If time exceeds 30 days of abeyance, the provider will be placed in summary suspension IAW AFI 44-119, paragraph 9.16. Until the license, registration or certification is renewed. Issuing agency grace periods are not honored by the AFMS when renewing a license, certification, or registration. Privileging Requirements. The credentials of the CP shall meet the privileging requirements as outlined in the most current version of AFI 44-119, Medical Quality Operations The CP shall furnish the MTF Credentialing Office with copies of valid, current, active, unrestricted medical licenses, registrations, education training certificates, and/or certifications in accordance with AFI 44-119, Medical Quality Operations. In addition, copies of board certifications are required, if applicable. The CP shall continuously maintain at least one active licensure or authorizing document from any U.S. jurisdiction throughout the life of this Contract. The CP shall be responsible for primary source verifications of all licenses and documents. Applications for privileges shall be screened through the National Practitioners Data Bank (NPDB), Healthcare Integrity and Protection Data Bank (HIPDB) by the CP. The CP shall also screen through The Department of Health and Human Services (DHHS) at http://exclusions.oig.hhs.gov/ and TRICARE sanction listing at www.tricare.osd.mil/. The listings must be reviewed prior to entering into Contract or hiring CP for the position. Professional staff appointments and clinical privileges are specific to the facility and the provider. An Initial Medical Staff appointment will be granted to all newly accessed CP for a period of twelve (12) months. During this period, the CP performance will be under close review by their assigned clinical supervisor for clinical competence as well as for compliance with the facility's policies, procedures, bylaws, and code of professional conduct. Thereafter, the CP privileges will be reviewed/ evaluated for an Active Medical Staff appointment, which is granted for a period of two (2) years. Healthcare professionals who fail to maintain a license, registration or certification will be removed from independent practice and privileged providers will be placed in abeyance. If time exceeds thirty (30) days of abeyance, the provider will be placed in summary suspension IAW AFI 44-119, Medical Quality Operations, (Paragraph 9.16.) until the license, registration or certification is renewed. Issuing agency grace periods are not honored by the AFMS when renewing a license, certification, or registration. 5) Place of Performance: These services would be performed at ) at San Antonio Texas Department of Defense Military Treatment Facilities (MTF). This survey is to determine the capability of potential sources and the availability of commercial sources and services pursuant to FAR Part 10. This notice is for informational purposes only and does not constitute an Invitation for Bid or Request for Proposal, and is not to be construed as a commitment by the Government. The Government will not reimburse contractors for any cost incurred for their participation in this survey. Large and small businesses are encouraged to participate in this survey. Prospective vendors are cautioned that if you cannot provide Personal Services for Dental Researcher/ Comprehensive General Dentist, please do not respond to this notice. In the event a prospective vendor is selected for an award, the selected vendor must be registered in the Systems for Awards Management website at www.sam.gov.
