SOURCES SOUGHT VA248-14-I-1597 James A. Haley Department of Veterans Affairs Medical Center Tampa, FL Fire Extinguisher Inspection, Testing & Maintenance The Department of Veterans Affairs, Veterans Integrated Service Network (VISN 8), James A. Haley Veterans Hospital, Tampa, FL is currently seeking information from firms, Service Disabled Veteran Owned Small Business, Veteran Owned Small Business, Hub zone, 8(A), Woman Owned Small Business OR Small Business with the ability to provide these following services and requirements listed below: Statement of Work (SOW) A. GENERAL INFORMATION 1. Title of Project: Portable Fire Extinguishers Inspection, Testing, and Maintenance 2. Scope of Work: The contractor shall provide all labor, supervision and all other resources required to deliver the requirements stated herein, except as may otherwise be specified in this statement of work. 3. Background: The National Fire Protection Association (NFPA) requires that portable fire extinguishers be inspected, tested and maintained in accordance with NFPA 10. The VA adopts the latest edition of the NFPA Codes. This service is also required per The Joint Commission and OSHA. 4. Performance Period: The anticipated period of performance is 1 year with options for renewal for 4 additional years. Annual Maintenance is to be performed in June of each year. 5. Type of Contract: A firm-fixed-price agreement. B. CONTRACT AWARD MEETING 1.Contractor will attend a pre-work orientation meeting prior to the commencement of work on site. The VA will schedule this meeting and it will include discussion of the following topics: a)Fire and Safety b)Infection Control c)Disaster procedures d)Other 2.VA will provide information to the contractor regarding these topics and will document the meeting. 3.The Contractor will be responsible to ensure that Contractor employees coming to the work site will receive the information required above. 4.The Contractor will be responsible to ensure Contractor employees providing work on this contract are fully trained and completely competent to perform the required work. 5.Contractor must possess and furnish written proof of a fire extinguisher manufacturer factory service certification and/or National Association of Fire Equipment Distributors Portable Fire Extinguisher Technician certification relevant to the type of work performed. C. GENERAL REQUIREMENTS 1.All work is to be done between the hours of 7:00 AM and 5:00 PM, Monday through Friday unless otherwise specified. 2.Work dates shall be scheduled with VA COR. 3.The invoice must include the building(s) inspected, work performed, dates work performed, testing interval, and components tested and maintenance performed. 4.Contractor will report to the VA Safety Office prior to starting any work to sign in and obtain a copy of the portable fire extinguisher inventory log. 5.A VA Safety Office employee will assist or provide entry key for locked areas. VA will provide keys to contractor for majority of areas. Those locked areas requiring VA assistance will require approximately one-half hour advance notice from the contractor to the VA. VA will furnish no tools, supplies or employees to assist with inspection and test work. D. SPECIFIC MANDATORY TASK 1. Provide all labor, time, equipment, materials and supervision for the maintenance, testing, and inspection of approximately 1,230 fire extinguishers (numbers may vary depending on construction) throughout James A. Haley VA Hospital, 13,000 Bruce B. Downs Blvd., Tampa, FL 33612. An inventory of the extinguishers is attached. a.Types of fire extinguishers including approximate quantity: 1)2.5- pound type ABC dry chemical - 110 2)5-pound type ABC dry chemical - 141 3)6-pound type ABC dry chemical - 38 4)10-pound type ABC dry chemical - 710 5)20-pound type ABC dry chemical - 1 6)5-pound type Carbon Dioxide - 87 7)10-pound type Carbon Dioxide - 95 8)20-pound type Carbon Dioxide - 3 9)6-liter type K extinguisher - 9 10)2.5-gallon Water Extinguisher - 21 11)50-pound type Purple K extinguisher - 1 b.Provide unit pricing for annual maintenance for each type of extinguisher. c.Provide unit pricing for internal examination (6 year maintenance) including associated maintenance, parts, and recharging for each type of fire extinguisher. (see attached inventory for due dates) 1)5-pound type ABC dry chemical - 20 2)10-pound type ABC dry chemical - 54 d.Provide unit pricing for hydrostatic testing (5 year or 12 year depending on cylinder type) including associated internal examination, maintenance, parts, and recharging for each type of fire extinguisher. (see attached inventory for due dates) 1)5-pound type ABC dry chemical - 5 2)10-pound type ABC dry chemical - 18 3)5-pound type Carbon Dioxide - 14 4)10-pound type Carbon Dioxide - 4 e.Provide unit pricing for recharging of each type of fire extinguisher. f.Provide unit pricing for disposal of all extinguishers that are deemed not serviceable or out of date of service. E. SCHEDULE OF DELIVERABLES 1. Annual maintenance is to be conducted every June following all requirements listed in the latest edition of NFPA 10. The annual maintenance is to be conducted by a technician certified in accordance with the latest edition of NFPA 10. The annual maintenance is to include a dusting/cleaning/wipe down of each unit and re-installation of tamper seals and contractor provided tags dated with the current year. Perform the six year and 12 year service as necessary. a)This maintenance will be documented on a mutually agreed upon tags (contractor furnished) provided on each extinguisher and will include the month/day/year and initials of the technician. b)The inspection will also be annotated on the inventory log provided by the VA verifying that the extinguisher received the required annual maintenance. 2. Contractor shall perform internal inspections and/or hydrostatic testing as required (See attached inventory). Contractor shall provide and install temporary fire extinguishers in place of those units being serviced. 3. Fire Extinguishers that have been discharged and require recharging will also be included in this contract. F. CONFIDENTIALITY AND NONDISCLOSURE 1. The preliminary and final deliverables, and all associated working papers, application source code, and other material deemed relevant by VA which have been generated by the contractor in the performance of this task order, are the exclusive property of the U.S. Government and shall be submitted to the COR at the conclusion of the task order. 2. The COR will be the sole authorized official to release, verbally or in writing, any data, draft deliverables, final deliverables, or any other written or printed materials pertaining to this task order. No information shall be released by the contractor. Any request for information relating to this task order, presented to the contractor, shall be submitted to the COR for response. 3. Press releases, marketing material, or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the COR. 4. It is prohibited for the contractor and any representatives to access any VA communications networks. C&A requirements do not apply. Special Requirements: All work shall be performed by fully qualified personnel. Employees must be aware that work is in a hospital environment and courtesy must be extended to patients and visitors. Work shall be coordinated to minimize impact to hospital operations. The applicable North American Industry Classification System Code (NAICS) for this procurement is 325998. To be considered a small business under this code the SBA size standard must not exceed $7.0M.
