Description
Sources Sought The Department of Veterans Affairs, Veterans Integrated Service Network (VISN 8), James A. Haley Veterans Hospital, Tampa, FL is currently seeking firms, Service Disabled Veteran Owned Small Business, Veteran Owned Small Business, Hub zone, 8(A), Woman Owned Small Business OR Small Business with the ability to provide the items listed below. A.GENERAL INFORMATION 1. Title of Project: Aquillion 64 VELOCT ENCORE upgrade with AIDR 3D for Toshiba Aquillion 64 CT Systems. 2. Scope of Work: The contractor shall provide all resources (labor, materials/parts, tools, equipment, and transportation) necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. The vendor shall deliver, install, and train JAHVH staff on the proper use of the Aquillion 64 VELOCT ENCORE upgrade with AIDR 3D for Toshiba Aquillion 64 CT System. at the James A. Haley Veterans Hospital. The system shall be fully operational at the conclusion of the work. 3. Performance Period: Work at the Government site shall not take place on Federal holidays unless directed by the CO. All work shall be performed Monday through Friday 7:00 AM to 5:00 PM. 4. Type of Contract: Time and materials B. GENERAL REQUIREMENTS 1.The VA shall not provide service manuals or service diagnostic software to the Contractor. The Contractor shall obtain, have on file, and make available to its FSE(s), all operational and technical documentation (e.g., operational and service manuals, schematics, and parts lists) which are necessary to meet the performance requirements of this contract. The location and listing of the service data manuals, by name, and/or the manuals themselves shall be provided to the CO upon request. Any charges for parts, manuals, tools, or software required to successfully complete any service required are included within this contract and its agreed upon price unless specifically stated in writing otherwise. 2.The Contractor will provide all labor and parts required for the installation of the aforementioned system and bring said system to full manufacturer's specifications. All parts must meet or exceed manufacturer's factory specifications to maintain compatibility with systems presently in place and with future performance/reliability upgrades. 3. At the conclusion of the installation, the Contractor will provide a written service report indicating the dates of service, the model, serial number, and barcode/ID number of the equipment installed the name of the service representative, the hours worked description of the tasks and the services performed. The reports will be delivered to COTR for signature when work is complete; a copy will be sent to Biomedical Engineering. During non-standard hours, reports will be taken to Police Dispatch for signature. C. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. D. GOVERNMENT RESPONSIBILITIES The government will make the equipment available in accordance with the established schedule for the work. CONTRACTOR EXPERIENCE REQUIREMENTS Each respondent must have an established business, with an office and full time staff to include a "fully qualified" field service engineer (FSE). The FSE(s) shall be authorized by the Contractor to perform the maintenance services. Businesses responding shall indicate their socio-economic status for example: small business, disadvantaged small business, HUBzone, woman owned business, veteran owned small business, service disabled veteran owned small business, etc. The Small Business Administration website has the small business defined: http://pro-net.sba.gov/cgi-bin/closemesba.pl?TO=https%3A%2F%2Feweb1%2Esba%2Egov%2 Fnaics%2F. Respondents shall indicate their status as a foreign-owned or foreign-controlled firm and any contemplated use of foreign national employees on this requirement. The NAICS code for this sources sought is 811219, Other Electronic and Precision Equipment Repair and Maintenance. This has a size standard of $19 million. The government reserves the right to consider a small business set-aside based on responses to these sources sought. All information submitted by respondents to this sources sought synopsis is voluntary. This sources sought notice is not to be construed as a commitment by the Government, nor will the Government reimburse any costs associated with the submission of information in response to this notice. Respondents will not individually be notified of the results of any Government assessments. The Government's evaluation of the capability documentation received will factor into any possible forthcoming solicitation.
