- ***The purpose of this amendment is to provide the correct version of Attachment 0001 - PWS, dated 21 July 2014*** This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a combined synopsis/solicitation for ISO 9001:2008 auditing in support of Joint Munitions Command (JMC) headquarters in accordance with the Performance Work Statement (Attachment 0001), dated 21 July 2014. This combined synopsis/solicitation is prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice and in accordance with simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the solicitation only; quotes are being requested and a written solicitation will not be issued. This Request for Quote (RFQ) is issued as a Total Small Business Set-Aside. The Government anticipates award of a single Indefinite Delivery Indefinite Quantity (IDIQ) type purchase order. Solicitation W52P1J-14-T-DB08 will be firm-fixed price, in accordance with the terms and conditions included in Attachment 0002, Clauses and Provisions. An ISO 9001:2008 surveillance audit or an ISO 9001:2008 certification audit in support of the JMC Quality Directorate (JMC QA) shall serve as the guaranteed minimum. Award will be made to the lowest price offeror that provides the minimum material submission requirements as outlined in section 4.1 of Attachment 0001. Pricing shall be submitted on Attachment 0004, Price Evaluation Sheet. Contractors shall submit prices for all services listed below to be considered for award: Ordering Period 1 (Date of Award - 365 days) - JMC QA ISO 9001:2008 surveillance audit or a certification audit (whichever your firm has the capability to provide) - HQ JMC ISO 9001:2008 pre-assessment audit Ordering Period 2 (366 days - 731 days) - JMC QA ISO 9001:2008 surveillance audit - HQ JMC ISO 9001:2008 pre-assessment audit - HQ JMC ISO 9001:2008 registration audit The following NAICS code and small business size standard apply: NAICS: 541614 Size Standard: $14,000,000 The following attachments are hereby incorporated by reference: Attachment 0001 - Purchase Description Attachment 0002 - Contract Clauses/Provisions Attachment 0003 - DFARS Tax Liability Attachment 0004 - Price Evaluation Sheet Quotes shall be submitted via e-mail by 12:00 PM Central Time on 8 August 2014 to the following individual: Mr. David Burke E-Mail: david.burke45.civ@mail.mil Phone: (309)782-7532 As referenced in the addendum to FAR 52.212-1, Instruction - Commercial Items, the following document shall be included with your quote: - A letter from its accreditation body certifying that the accreditation body is in compliance with ISO/IEC 17011:2004 - A valid certificate of accreditation from its accreditation body which demonstrates compliance with ISO/IEC 17021 - A signed copy of Attachment 0003 is required with your submission - A completed copy of Attachment 0004 is required with your submission.
