- This is a NOTICE OF INTENT TO SOLE SOURCE for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only acknowledgment and a written solicitation will not be issued. This contract is being procured a sole source basis due to the fact that Olympus is the manufacturer of the parts. No other known company manufactures their parts. This is being procured on an unusual and compelling basis as a result of this being the only Ultrasound Bronchoscope at the facility and is used to diagnose and evaluate therapy treatments for patients. The contract duration is upon completion of repairing the Ultrasound Bronchoscope to manufacturer's specifications and industry standard. This requirement is unrestricted-insufficient small business for commercial items under the North Atlantic Industrial Classification System (NAICS) 811219. Department of Veterans Affairs VISN 10 Contracting - NBC 4100 West Third Street Dayton, OH 45428 STATEMENT OF WORK A.GENERAL INFORMATION 1.Title of Project: Repair of Olympus BF-UC160F-OL8 Ultrasound Endoscope, SN 1700457, EE 40778. 2.Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. The contractor is to provide the services to repair the endoscope in order to bring it to full operational status according to Manufacturer's specifications. 3.Background: This is the only Ultrasound Endoscope at the facility and as such it is pertinent that this repair be expedited to ensure minimal interruption in patient care. 4.Performance Period: The work shall begin within _1__ calendar day of award, unless otherwise specified. 5.Type of Contract: Firm Fixed Price B. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor.
