Description
This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested and a written Solicitation will not be issued. Solicitation number VA249-14-Q-0791 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular. This requirement is Open Market - Unrestricted. The NAICS is 811219 and the small business size standard is $19 Million. Contract Type: The government anticipates awarding a Firm Fixed Price award. The bases of award will be lowest price quote. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforms to the solicitation requirements. Description of Services: The Lexington VA Medical Center is seeking Service/Maintenance Contract for Philips Imaging Machines with a Base Year plus (1) one-year option period to renew. The Contractor will provide service/maintenance which may consist of calibration, cleaning, oiling, adjusting, replacing parts, (without additional cost to the Government), and maintain the equipment, including all intervening calls necessary between regular services and calibrations. All required parts shall be furnished by the Contractor. Contractor shall maintain the equipment in accordance with the Conformance Standards Section. See attached Statement of Work (SOW) for complete details. EQUIPMENT: Equipment to be serviced/maintained includes but not limited to: 1. Philips 1.5T Achieva MRI System, Serial #32118, VAMC ID# 54877, Site# 45009694, and System includes Gradients, Magnet, Coils, RF Amplifier, and Patient Table 2. Spectris Medrad Solara Injector (EE# 54884, S/N 44387), Control Workstation (EE# 54883), and Physician Workstation (EE# 54879). 3. Philips Integris Allura S/P cardiovascular x-ray system (ID# 38974, site# 533567) including electronic cabinets, x-ray generator, operator console, monitors, 4. Medrad Provis Injector (EE# 37336, S/N 100858) and all glassware including the x-ray tube and the Image Intensifier, 5. Philips Gemini TF 16 w/TOF to include the UPS (Site# 57425194, S/N 7584), EBW Workstation (Site# 57425195, S/N 68504), 6. Medrad Stellant DX Injector (Site# 58063916, S/N 39495) IE33 Ultrasound units (EE# 44428, S/N 02R7F8) and (EE# 49438, S/N 02XDF6) Period/Place of Performance: Lexington VAMC, Cooper Drive Division, 1101 Veterans Drive, Lexington KY 40502. Base Year - October 1, 2014 thru September 30, 2015 with (1) one-year option period to renew through September 30, 2016. The following clauses and provisions apply to this solicitation: 52.204-7, Central Contractor Registration - title is now System for Award Management [SAM]. 52.212-1, Instructions to Offerors--Commercial 52.212-3, Offeror Representations and Certifications-Commercial Items 52.212-4, Contract Terms and Conditions-Commercial Items 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items 52.217-8, Option to Extend Services The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 60 days. 52.217-9, Option to Extend the Term of the Contract a) The Government may extend the term of this contract by written notice to the Contractor within 60 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. Submission of Quote: The Offeror shall submit their quote on company letterhead and shall include unit price, total, unit quantity, terms of any express warranty, unit price, overall total price, applicable shipping charges and item description as specified above, as offered discounts, proposed delivery time, name, address, and telephone number of the offeror, firm's DUNS# and ORCA document in SAM at www.sam.gov. Complete copy of 52.212-3 Offerors Representations and Certifications- Commercial Items, and ORCA document. The offeror shall provide sufficient evidence that they possess adequate resources, capability, experience, responsibility and integrity to meet the technical capabilities to comply with the requirements of the resulting contract. The offeror shall provide past performance evidence. This assessment is based on the offeror's record of relevant and recent past performance information that pertains to services outlined in the solicitation requirements. The offeror shall provide evidence that all workers (FSE, engineers, technicians and helpers) must have certification/documentation as evidence of their degree of training and skills in order to perform the requirements in the solicitation. Certification/documentation must be submitted at the time of offer. All quotes received without this documentation will not be considered.
