- This is a sources sought only; it is not a request for quotations or offers. This procurement requirement is to determine whether any small business can provide can provide these services listed below: CLASSIFICATION CODE: Q - Medical Services; (Q515- Medical Service/Pathology) NAICS CODE: 621 - Pathologist Services in Support of VAMC Lexington; 621111- (Pathologists) Network-9 Contracting Office on the behalf of the Lexington VA Medical Center, 1101 Veterans Drive, Lexington, KY., has developed this Request for Information (RFI)/Sources Sought Notice to solicit market research information from industry to assist the Government with analysis of a Government requirement and provide commercial insight on performance-based solutions, best practices, possible contractual arrangements, and potential issues. This RFI is to determine the availability of Small Business (preferably Service-Disabled and/or Veteran-Owned Owned) entities within the Lexington area. THIS IS A REQUEST FOR INFORMATION ONLY. It is not a Request for Proposal, a Request for Quotation, an Invitation for Bids, a Solicitation, or an indication that Network-9 Contracting Office (NCO-9) will contract for the items contained in the RFI / Sources Sought Notice. This RFI/Sources Sought Notice is part of a Government market research effort to determine the scope of industry capabilities and interest, in accordance with Federal Acquisition Regulation (FAR) Part 15.201(e), response to this RFI / Sources Sought Notice are not offers and cannot be accepted by the Government to form a binding contract. Responses to this RFI / Sources Sought Notice are strictly voluntary and are consider as information only. The Government will not pay respondents for information provided in response to this RFI / Sources Sought Notice. Responses to this RFI / Sources Sought Notice will not be returned and respondents will not be notified of the result of the review. If a solicitation is issued, it will be announced on the Federal Business Opportunities Website http://www.fbo.gov at a later date, and all interested parties must respond to that solicitation announcement separately from any response to this announcement. This RFI / Sources Sought Notice do not restrict the Government's acquisition on a future solicitation. Network-9 Contracting Office (NCO-9) and Lexington VA Medical Center are conducting market research for a Contractor to furnish Pathology Services procedures for the Department of Veterans Affairs (VA) at the 1101 Veterans Drive, Lexington, KY.. Please review the draft Performance Work Statement (PWS) provided below before responding to the market research questions as the Government desires that respondents offer their experience and recommendations on the following questions and issues related to this requirement. When addressing these items, respondents should consider the size and scope of the requirement described herein. PERFORMANCE WORK STATEMENT: PERFORMANCE WORK STATEMENT Pathologist Services in Support of VAMC Lexington Transfusion Medicine (Blood Bank) 1. Introduction & Scope of Work There exists a regulatory mandate for pathologist medical director support of the Blood Bank/Transfusion Medicine services of Pathology & Laboratory Medicine Service. This basic need must be met by an appointed Blood Bank Pathologist Medical Director. The contractor shall provide Pathologist Clinical/Technical Services in support of VAMC Lexington Transfusion Medicine (VA Blood Bank) Pathology & Laboratory Medicine Service 1101 Veterans Drive Lexington, KY. Coverage and responsibilities are estimated to require approximately 21 hours per month (252 hours per year). The Pathologist serves as Medical Director of the Blood Bank and must be fully credentialed by VAMC Lexington's official credentialing process prior to performing the duties of the position. The contract will be from date of award for one base year and four one-year options. 2. Requirements The Pathologist Medical Director of the Blood Bank assumes the following responsibilities: 1.Promotes and supports the principles and activities of continuous quality improvement 2.Provides general oversight and control for the Blood Bank operations and transfusion & tissue practices throughout the facility 3.Serves as the responsible head of the Blood Bank in all dealings with accrediting agencies 4.Provides consultation to other physicians regarding appropriate transfusion and tissue related practice 5.Assists the Blood Bank Supervisor and Quality Coordinator in the development and maintenance of the Quality Program 6.Reviews all self-assessments and monitors 7.Reviews quarterly Quality Improvement Report Form (QIRF) data assessments 8.Reviews the annual Quality Plan 9.Reviews and approves all medical and technical policies, processes, and procedures in the Blood Bank; although he or she has the ability to delegate responsibility to a qualified physician. 10.Serves as chairman of the hospital Transfusion & Tissue Review Committee 11.Provides justification and pre-approval on exceptions to policies, processes, and procedures warranted by clinical situations. 12.Responsible for transfusion medicine related "look-back" follow up and notification. 13.Responsible for transfusion medicine related transmitted disease notification 14.Responsible for tissue related "look-backs" and recall notification 15.Reviews CAP proficiency testing results 16.Reviews tissue vendor requests 3. TRAINING Contractor personnel complete all mandatory training per VAMC policy. 5. DELIVERY Invoicing, in the form of contract specified charge amount per billable hours, shall occur monthly with hourly coverage amounts recorded in a time and attendance log maintained by the VAMC P&LMS Blood Bank.
